Description
Overview of the medicine
Nocpres 200 mcg Tablet contains Desmopressin, a synthetic analogue of the natural posterior pituitary hormone, vasopressin. It is used to treat conditions involving excessive urine production, such as nocturnal enuresis (bedwetting), central diabetes insipidus, and nocturnal polyuria.
Uses & Indications
Dosage
Adults
For nocturnal polyuria, initially 200 mcg at bedtime, dose may be increased to 400 mcg if needed. For diabetes insipidus, individualize dosage, typically 100-200 mcg 1-3 times daily.
Elderly
Lower starting dose and careful monitoring for hyponatremia. Not recommended for patients over 65 years with nocturnal polyuria.
Renal_impairment
Use with caution. Not recommended in severe renal impairment (CrCl < 50 mL/min).
How to Take
Take the tablet sublingually (under the tongue) without water, allowing it to dissolve completely. Do not chew or swallow the tablet whole. Administer at bedtime for nocturnal indications.
Mechanism of Action
Desmopressin acts as a selective agonist for V2 vasopressin receptors, primarily in the renal collecting ducts. This increases the reabsorption of water, leading to a decrease in urine output and an increase in urine osmolality, thus concentrating the urine.

Onset
Antidiuretic effect usually starts within 1 hour.
Excretion
Excreted primarily via the kidneys (renal excretion) in unchanged form.
Half life
The elimination half-life is approximately 2.5 to 3.5 hours.
Absorption
Oral bioavailability of desmopressin is low (0.08-0.16% for tablets), but sublingual absorption is better for the higher potency formulation. Peak plasma concentrations are reached within 0.5 to 2 hours.
Metabolism
Minimal hepatic metabolism. Primarily degraded by peptidases.
