Overview of the medicine
Norfen-SR (Naproxen) is a non-steroidal anti-inflammatory drug (NSAID) used for the relief of pain, inflammation, and stiffness caused by various conditions such as arthritis, gout, and menstrual cramps. The SR formulation allows for once-daily dosing.
Uses & Indications
Dosage
Adults
500 mg or 750 mg once daily. Max 1000 mg/day.
Elderly
Lower effective dose should be used, monitor for adverse effects, especially renal function.
Renal_impairment
Use with caution; dose reduction may be necessary in moderate to severe impairment. Contraindicated in severe renal failure.
How to Take
Take orally, preferably with food or milk to minimize gastrointestinal upset. Swallow the sustained-release tablet whole; do not crush, chew, or break it.
Mechanism of Action
Naproxen inhibits cyclooxygenase (COX-1 and COX-2) enzymes, leading to a reduction in prostaglandin synthesis. Prostaglandins are mediators of inflammation, pain, and fever.

Onset
Analgesic effect begins within 1 hour, anti-inflammatory effects may take longer to develop.
Excretion
Excreted mainly in the urine (approximately 95%) as unchanged drug and metabolites; a small amount is excreted in feces.
Half life
Approximately 12-17 hours.
Absorption
Well absorbed from the gastrointestinal tract. Peak plasma concentrations for SR formulation are reached in 4-7 hours.
Metabolism
Primarily metabolized in the liver (via CYP enzymes) by O-demethylation, followed by conjugation.

Drug Interactions
SSRIs

Reduced antihypertensive effect and increased risk of renal impairment.
Anticoagulants (e.g., Warfarin)
Increased risk of bleeding.

Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include drowsiness, heartburn, nausea, vomiting, epigastric pain. Management is supportive and symptomatic; gastric lavage and activated charcoal may be considered.
Pregnancy & Lactation
Avoid in the third trimester of pregnancy due to risk of premature closure of the fetal ductus arteriosus. Use with caution during lactation; Naproxen is excreted in breast milk.
