Description
Overview of the medicine
Novetor (Olmesartan Medoxomil) is an angiotensin II receptor blocker (ARB) used to treat high blood pressure (hypertension) in adults and children 6 years and older. It helps to lower blood pressure, which can reduce the risk of strokes and heart attacks.
Uses & Indications
Dosage
Adults
Initial dose 20 mg once daily. May be increased to 40 mg once daily if needed. Maximum recommended dose is 40 mg once daily.
Elderly
No initial dosage adjustment is typically required, but monitoring of renal function is advised.
Renal_impairment
Initial dose 10 mg once daily for moderate to severe renal impairment. Careful monitoring is necessary.
How to Take
Novetor tablets can be taken orally once daily with or without food.
Mechanism of Action
Olmesartan Medoxomil is a selective AT1 subtype angiotensin II receptor antagonist. It blocks the vasoconstricting and aldosterone-secreting effects of angiotensin II by selectively blocking the binding of angiotensin II to the AT1 receptor in many tissues, including vascular smooth muscle and the adrenal gland. Its action is independent of the pathway for angiotensin II synthesis.
Pharmacokinetics
Onset
Antihypertensive effect generally evident within 2 weeks, maximal effect achieved within 8 weeks.
Excretion
Approximately 35-50% excreted in urine and the remainder in feces via bile.
Half life
Approximately 13 hours.
Absorption
Rapidly and completely absorbed from the GI tract after oral administration, bioavailability is approximately 26%. Peak plasma concentrations reached within 1-2 hours.
Metabolism
Not extensively metabolized in the liver. Medoxomil group is hydrolyzed to the active olmesartan.
Side Effects
Contraindications
- Pregnancy (second and third trimesters)
- Biliary obstruction
- Concomitant use with aliskiren in patients with diabetes mellitus or renal impairment (GFR <60 ml/min/1.73 m²)
- Hypersensitivity to Olmesartan Medoxomil or any component of the formulation
Drug Interactions
NSAIDs
Reduced antihypertensive effect and increased risk of renal impairment, especially in elderly or dehydrated patients.
Lithium
Increased serum lithium concentrations and toxicity. Monitor lithium levels.
Aliskiren
Contraindicated in diabetic patients due to increased risk of renal impairment, hyperkalemia, and hypotension.
Diuretics
Increased risk of hypotension, especially with initial doses.
Potassium-sparing diuretics/Potassium supplements
Increased risk of hyperkalemia. Monitor serum potassium.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
The most likely manifestation of overdose is hypotension. Treatment should be symptomatic and supportive, including close monitoring of vital signs and hydration. Olmesartan is not dialyzable.
Pregnancy & Lactation
Novetor is contraindicated during the second and third trimesters of pregnancy due to fetal toxicity. Discontinue as soon as pregnancy is detected. It is not recommended during lactation due to potential adverse effects on the nursing infant.
