Novonil 20 mg Tablet contains Olanzapine, an atypical antipsychotic medication used to treat schizophrenia and bipolar disorder. It helps to restore the balance of certain natural substances in the brain.
Uses & Indications
Dosage
Adults
Schizophrenia: Initially 5-10 mg once daily, adjusted to 10-20 mg/day. Bipolar mania: Initially 10-15 mg once daily, adjusted to 5-20 mg/day. Prevention of recurrence in bipolar disorder: 10 mg/day.
Elderly
Lower starting dose (e.g., 5 mg/day) and slower titration may be considered, especially for those with factors that could result in slower metabolism.
Renal_impairment
No specific dose adjustment is routinely required, but caution is advised in severe cases.
How to Take
Take orally, once daily, with or without food. The tablet should be swallowed whole with water. It is advisable to take it at the same time each day.
Mechanism of Action
Olanzapine acts as an antagonist at multiple receptor sites, including dopamine D1, D2, D3, D4, and D5 receptors, serotonin 5-HT2A, 5-HT2C, 5-HT3, and 5-HT6 receptors, alpha1-adrenergic receptors, and histamine H1 receptors. Its efficacy in schizophrenia and bipolar disorder is thought to be mediated through a combination of dopamine and serotonin antagonism.
Pharmacokinetics
Onset
Antipsychotic effects may take several days to weeks to become fully apparent. Sedation can occur within hours.
Excretion
Mainly excreted in urine (approximately 57%) and feces (approximately 30%). Less than 7% is excreted as unchanged drug.
Half life
Approximately 21-54 hours (mean 33 hours) in healthy subjects. It may be prolonged in elderly or those with hepatic impairment.
Absorption
Readily absorbed after oral administration, reaching peak plasma concentrations in 5-8 hours. Food does not affect the rate or extent of absorption.
Metabolism
Extensively metabolized in the liver, primarily by glucuronidation and cytochrome P450 enzymes (CYP1A2 and CYP2D6). N-demethylation and 10-hydroxylation are the major metabolic pathways.

- Hypersensitivity to Olanzapine or any component of the formulation
- Patients with known narrow-angle glaucoma

May inhibit olanzapine metabolism, increasing plasma levels.
Carbamazepine
May increase olanzapine clearance, reducing plasma levels.
Antihypertensives
Potentiation of orthostatic hypotension.

Symptoms include drowsiness, slurred speech, tachycardia, extrapyramidal symptoms, and respiratory depression. Management is supportive, including maintaining airway, oxygenation, and hydration.

Pregnancy Category C. Use only if potential benefit justifies potential risk to the fetus. Not recommended during breastfeeding as it is excreted in breast milk.
