Description
Overview of the medicine
Noxarin-2000 Anti-Xa Injection contains Enoxaparin Sodium, a low molecular weight heparin (LMWH), used to prevent and treat various thromboembolic disorders. It works by inhibiting coagulation factors, primarily Factor Xa.
Uses & Indications
Dosage
Adults
VTE prophylaxis: 20-40 mg SC once daily. DVT/PE treatment: 1 mg/kg SC every 12 hours or 1.5 mg/kg SC once daily. Unstable Angina/NSTEMI: 1 mg/kg SC every 12 hours.
Elderly
No dose adjustment generally required, but monitor for renal function. For patients >75 years with STEMI, use half the bolus dose.
Renal_impairment
Severe renal impairment (CrCl < 30 mL/min): Adjust dose. VTE prophylaxis: 20 mg SC once daily. DVT/PE treatment: 1 mg/kg SC once daily.
How to Take
Administer by subcutaneous injection into the abdominal wall, alternating between the left and right anterolateral or posterolateral abdominal wall. Do not inject intramuscularly. Do not expel the air bubble from the syringe before injection.
Mechanism of Action
Enoxaparin potentiates the activity of antithrombin III, selectively inhibiting Factor Xa and, to a lesser extent, Factor IIa (thrombin). This results in anticoagulant effects, preventing clot formation and growth.
Pharmacokinetics
Onset
1-2 hours after subcutaneous injection.
Excretion
Primarily renal, with approximately 40% of the dose excreted as active and inactive fragments in urine.
Half life
Approximately 4.5 to 7 hours after a single SC dose.
Absorption
Rapid and complete after subcutaneous (SC) administration, with peak anti-Xa activity achieved in 3-5 hours.
Metabolism
Undergoes desulfation and depolymerization in the liver to lower molecular weight species.
Side Effects
Contraindications
- •Active major bleeding or conditions with a high risk of uncontrolled hemorrhage
- •History of heparin-induced thrombocytopenia (HIT) with or without thrombosis
- •Hypersensitivity to enoxaparin, heparin, or pork products
- •Acute bacterial endocarditis
- •Spinal or epidural anesthesia (risk of hematoma)
Drug Interactions
Warfarin
Increased risk of bleeding. Monitor INR closely.
NSAIDs (e.g., Aspirin, Ibuprofen)
Increased risk of bleeding. Concomitant use should be with caution.
Antiplatelet agents (e.g., Clopidogrel)
Increased risk of bleeding.
Other anticoagulants (e.g., unfractionated heparin, oral anticoagulants)
Increased risk of bleeding.
Storage
Store at room temperature (below 25°C). Do not freeze. Protect from light and moisture. Keep out of reach of children.
Overdose
Accidental overdose may lead to hemorrhagic complications. Protamine sulfate is the antidote. For 1 mg of enoxaparin, administer 1 mg of protamine sulfate intravenously.
Pregnancy & Lactation
Pregnancy Category B. Use during pregnancy only if clearly needed. Enoxaparin does not cross the placental barrier. Excretion into breast milk is minimal; generally considered safe during lactation, but caution is advised.
