Description
Overview of the medicine
Noxarin-4000 is an injectable anticoagulant containing Enoxaparin Sodium, a low molecular weight heparin (LMWH). It is used to prevent and treat various thromboembolic disorders by inhibiting the formation of blood clots.
Uses & Indications
Dosage
Adults
Varies by indication: e.g., DVT prophylaxis: 40 mg SC once daily or 30 mg SC twice daily. DVT/PE treatment: 1 mg/kg SC every 12 hours or 1.5 mg/kg SC once daily. Unstable angina/NSTEMI: 1 mg/kg SC every 12 hours.
Elderly
No routine dose adjustment needed for elderly unless severe renal impairment (CrCl < 30 mL/min), where dose reduction is recommended.
Renal_impairment
For CrCl < 30 mL/min: Dose reduction is required. For DVT prophylaxis, 30 mg SC once daily. For DVT/PE treatment, 1 mg/kg SC once daily.
How to Take
Administer by deep subcutaneous injection, preferably into the abdominal wall or anterior/posterior lateral thigh. Alternate injection sites. Do not inject intramuscularly. Do not rub the injection site after administration.
Mechanism of Action
Enoxaparin potentiates the inhibitory effect of antithrombin III on Factor Xa and, to a lesser extent, Factor IIa (thrombin). This selective inhibition prevents the formation and growth of blood clots.
Pharmacokinetics
Onset
Anti-Xa activity onset within 3-5 hours post-subcutaneous injection.
Excretion
Mainly renal, with approximately 40% of the dose excreted as active or inactive fragments in urine.
Half life
Approximately 4-5 hours (anti-Xa activity) in healthy individuals, can be extended in renal impairment to 7 hours.
Absorption
Rapid and almost complete absorption (approx. 100% bioavailability) after subcutaneous injection. Peak anti-Xa activity is achieved within 3-5 hours.
Metabolism
Primarily metabolized in the liver by desulfation and/or depolymerization into lower molecular weight species.
Side Effects
Contraindications
- •Active major bleeding or conditions associated with a high risk of uncontrolled bleeding (e.g., active gastric ulcer, hemorrhagic stroke).
- •History of heparin-induced thrombocytopenia (HIT) with or without thrombosis.
- •Hypersensitivity to enoxaparin, heparin, or pork products.
- •Spinal/epidural anesthesia or lumbar puncture within 24 hours of enoxaparin administration (for prophylactic doses).
Drug Interactions
Thrombolytic agents
Significantly increased risk of hemorrhage.
Warfarin, other oral anticoagulants
Increased risk of bleeding.
Aspirin, NSAIDs, antiplatelet agents (e.g., clopidogrel)
Increased risk of bleeding. Use with caution.
Storage
Store below 25°C (77°F), do not freeze. Protect from light. Keep out of reach of children.
Overdose
Accidental overdose may lead to hemorrhagic complications. The anticoagulant effects can be largely neutralized by the slow intravenous injection of protamine sulfate. The dose of protamine sulfate should be carefully calculated based on the administered enoxaparin dose and time since administration.
Pregnancy & Lactation
Pregnancy Category B. Use during pregnancy only if clearly needed and potential benefits outweigh risks. Enoxaparin does not cross the placental barrier. Minimal excretion into breast milk; considered compatible with breastfeeding with caution.
