Description
Overview of the medicine
Nystatin 100,000 units/ml oral suspension is an antifungal medication used to treat fungal infections of the mouth, throat, and esophagus, typically caused by Candida species (thrush). It works by inhibiting fungal growth.
Uses & Indications
Dosage
Adults
For oral candidiasis, 400,000-600,000 units (4-6 ml) four times daily. Swish in the mouth for several minutes before swallowing. Continue for at least 48 hours after symptoms disappear.
Elderly
Same as adult dose.
Renal_impairment
No dosage adjustment necessary due to minimal systemic absorption.
How to Take
Shake well before use. For oral candidiasis, place half of the dose in each side of the mouth. Hold in the mouth for as long as possible (several minutes) before swallowing, allowing contact with the affected areas. For infants and young children, apply to affected areas with a swab or dropper.
Mechanism of Action
Nystatin is a polyene antifungal that binds to sterols (primarily ergosterol) in the fungal cell membrane, leading to an increase in cell membrane permeability. This causes leakage of intracellular components and ultimately fungal cell death.
Pharmacokinetics
Onset
Symptomatic improvement typically within 24-72 hours.
Excretion
Primarily excreted unchanged in the feces.
Half life
Not well characterized due to minimal systemic absorption; estimated to be very short if any does enter systemic circulation.
Absorption
Nystatin is poorly absorbed from the gastrointestinal tract and mucous membranes, with negligible systemic absorption when administered orally.
Metabolism
Not extensively metabolized in the body due to poor absorption.
Side Effects
Contraindications
- Hypersensitivity to nystatin or any component of the formulation
Drug Interactions
None clinically significant for oral suspension due to poor absorption.
Nystatin is minimally absorbed systemically, therefore systemic drug interactions are unlikely.
Storage
Store at room temperature (20-25°C), away from light and moisture. Do not freeze.
Overdose
Oral doses of nystatin are generally well tolerated, even in large amounts, because of its poor absorption from the GI tract. Nausea, vomiting, and diarrhea may occur. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Nystatin is generally considered safe during pregnancy and lactation due to its minimal systemic absorption. Consult a doctor before use.

YesClinical Trials
Nystatin is an older drug; efficacy is well established through historical clinical use. Modern trials often focus on specific patient populations or novel formulations.
Lab Monitoring
- No specific lab monitoring is required for routine use of oral nystatin suspension due to minimal systemic absorption.
Doctor Notes
- Advise patients to maintain good oral hygiene and replace toothbrushes regularly to prevent reinfection.
- Ensure patients understand the 'swish and swallow' or 'swish and spit' technique for optimal contact with oral lesions.
- Consider underlying causes of candidiasis, especially in recurrent cases.
Patient Guidelines
- Shake bottle well before each use.
- Complete the full course of treatment, even if symptoms improve.
- Do not dilute the suspension.
- Avoid eating or drinking for at least 30 minutes after taking the dose, if treating oral thrush.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue with your regular dosing schedule. Do not double the dose to catch up.
Driving Precautions
Nystatin has no known effects on the ability to drive or operate machinery.
Lifestyle Advice
- Maintain good oral hygiene to prevent recurrence.
- Sterilize feeding bottles/pacifiers for infants.
- Control underlying conditions that predispose to candidiasis (e.g., diabetes).
