Description
Overview of the medicine
O Fruity Saline is an oral rehydration salt formulation with added fruity flavor, used to prevent and treat dehydration caused by diarrhea or excessive fluid loss. It replenishes essential electrolytes and fluids lost from the body.
Uses & Indications
Dosage
Adults
Acute diarrhea: 250-500 mL after each loose stool. Maintenance: 1-3 liters over 24 hours depending on dehydration severity.
Elderly
Same as adults, with careful monitoring.
Renal_impairment
Use with caution; consult a physician for severe renal impairment due to electrolyte balance concerns.
How to Take
Dissolve one sachet in 500 mL (half a liter) of boiled and cooled water. Stir well. Drink slowly. Do not add extra sugar or other ingredients. Use within 12 hours if kept at room temperature, or 24 hours if refrigerated.
Mechanism of Action
ORS works by utilizing the co-transport mechanism of glucose and sodium in the small intestine. Glucose helps sodium to be absorbed, and water follows passively, thereby restoring fluid and electrolyte balance.
Pharmacokinetics
Onset
Within 30-60 minutes for initial rehydration.
Excretion
Mainly through kidneys (urine); some through sweat and feces.
Half life
Not directly applicable as it's an electrolyte solution; components have varying half-lives depending on body needs and renal function.
Absorption
Rapidly absorbed from the gastrointestinal tract.
Metabolism
Glucose is metabolized for energy; electrolytes are absorbed and utilized as per physiological needs, not metabolized in the traditional sense.
Side Effects
Contraindications
- Severe dehydration requiring intravenous fluids
- Intestinal obstruction
- Paralytic ileus
- Perforation of the intestine
- Uncontrollable vomiting (where oral intake is not possible)
- Severe renal impairment (use with caution under medical supervision)
Drug Interactions
Diuretics
May require closer monitoring of electrolyte levels.
Other potassium-sparing agents
Risk of hyperkalemia with excessive ORS intake.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Excessive intake in individuals with normal renal function is usually well-tolerated. However, in individuals with impaired renal function, or if highly concentrated, hypernatremia or hyperkalemia can occur. Management involves discontinuation and symptomatic treatment.
Pregnancy & Lactation
Safe to use during pregnancy and lactation for rehydration.

Lab Monitoring
- Electrolyte levels (sodium, potassium, chloride) in severe dehydration or in patients with renal impairment.
Doctor Notes
- Emphasize proper preparation and administration, especially for pediatric patients.
- Monitor for signs of worsening dehydration or electrolyte imbalance.
Patient Guidelines
- Always use boiled and cooled water to prepare the solution.
- Do not add extra sugar or other ingredients.
- Discard unused solution after 12-24 hours.
- Seek medical attention if diarrhea or dehydration worsens.
Missed Dose Advice
Since ORS is administered as needed, there isn't a 'missed dose' in the traditional sense. Continue administration after each loose stool or as directed for dehydration.
Driving Precautions
None, does not affect ability to drive or operate machinery.
Lifestyle Advice
- Maintain good hygiene to prevent diarrhea.
- Ensure access to safe drinking water.
- Continue normal feeding for infants and young children during diarrhea.
