Overview of the medicine
Ofran 8 mg Injection contains Ondansetron, which is used to prevent and treat nausea and vomiting caused by chemotherapy, radiation therapy, and surgery.
Uses & Indications
Dosage
Adults
Chemotherapy-induced nausea and vomiting (CINV): 8 mg IV infused over 15 minutes, starting 30 minutes before chemotherapy. Post-operative nausea and vomiting (PONV): 4 mg IV/IM as a single dose at induction of anesthesia.
Elderly
No specific dose adjustment is generally required, but caution is advised in very elderly patients (over 75 years) due to increased half-life and potential for QT prolongation.
Renal_impairment
No dose adjustment is required for patients with renal impairment.
How to Take
Administer intravenously (IV) slowly over 2-5 minutes or intramuscularly (IM). For prevention of CINV, it can be given as an infusion over 15 minutes.
Mechanism of Action
Ondansetron selectively blocks serotonin 5-HT3 receptors located both peripherally on vagal nerve terminals and centrally in the chemoreceptor trigger zone, thereby preventing nausea and vomiting.

Onset
Within 30 minutes after intravenous administration.
Excretion
Approximately 5% excreted unchanged in urine, remaining as metabolites via renal and fecal routes.
Half life
Approximately 3-6 hours in adults.
Absorption
Rapid and complete absorption after oral administration, but for injection, it bypasses absorption and is immediately available.
Metabolism
Extensively metabolized in the liver, primarily by hydroxylation followed by glucuronide or sulfate conjugation.
Side Effects
Contraindications
- Known hypersensitivity to Ondansetron or any component of the formulation.
- Concomitant use with apomorphine (due to risk of profound hypotension and loss of consciousness).
Drug Interactions
Tramadol
May reduce the analgesic effect of tramadol.
Apomorphine
Concomitant use is contraindicated due to severe hypotension and loss of consciousness.
Drugs prolonging QT interval
Use with caution with other drugs known to prolong the QT interval (e.g., amiodarone, erythromycin, sotalol) as Ondansetron itself can prolong QT interval.
Phenytoin, Carbamazepine, Rifampicin
May increase the clearance of Ondansetron, reducing its plasma levels and potentially efficacy.

Symptoms of overdose include transient visual disturbances, constipation, severe hypotension, and a vagal episode with transient AV block. Treatment should be symptomatic and supportive.
