Description
Overview of the medicine
Olanza 10 mg Tablet contains Olanzapine, an atypical antipsychotic medicine used to treat schizophrenia and bipolar I disorder (manic or mixed episodes). It helps to restore the balance of certain natural substances in the brain.
Uses & Indications
Dosage
Adults
Schizophrenia: Initially 10 mg once daily. Target dose 10-20 mg/day. Bipolar mania: Initially 10-15 mg once daily. Target dose 10-20 mg/day. Maximum recommended dose 20 mg/day.
Elderly
A lower starting dose of 5 mg/day should be considered. Dose escalation should be done cautiously.
Renal_impairment
No routine dose adjustment is required, but caution is advised in patients with severe renal impairment.
How to Take
Take Olanza 10 mg Tablet orally once daily with or without food. Swallow the tablet whole with a glass of water. Do not chew or crush it.
Mechanism of Action
Olanzapine acts as an antagonist at multiple receptor sites, including dopamine D1, D2, D3, D4, serotonin 5-HT2A, 5-HT2C, 5-HT3, 5-HT6, alpha1-adrenergic, histamine H1, and muscarinic M1-M5 receptors. Its antipsychotic efficacy is primarily attributed to a combination of dopamine and serotonin antagonism.
Pharmacokinetics
Onset
Symptom improvement can begin within days to weeks; full therapeutic effects may take several weeks.
Excretion
Approximately 57% of the dose is excreted in urine and 30% in feces, predominantly as metabolites.
Half life
Approximately 21 to 54 hours (average 33 hours).
Absorption
Well absorbed orally; peak plasma concentrations reached in approximately 5-8 hours. Bioavailability is not affected by food.
Metabolism
Primarily hepatic, via direct glucuronidation and cytochrome P450-mediated oxidation (mainly CYP1A2, and to a lesser extent CYP2D6).
Side Effects
Contraindications
- Known hypersensitivity to olanzapine or any of its excipients.
- Patients with known narrow-angle glaucoma.
Drug Interactions
Fluvoxamine
May increase olanzapine levels; consider lower olanzapine starting dose.
Carbamazepine
May decrease olanzapine levels; dose adjustment may be necessary.
Dopamine Agonists
May antagonize the effects of olanzapine.
Alcohol/CNS Depressants
Increased risk of additive sedative effects.

Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include tachycardia, agitation, dysarthria, various extrapyramidal symptoms, and decreased level of consciousness ranging from sedation to coma. Management is supportive, involving maintaining an open airway, oxygenation, ventilation, and monitoring cardiovascular function. Activated charcoal may be useful.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Not recommended during breastfeeding as olanzapine is excreted in breast milk.
