Description
Overview of the medicine
Olmetor 10 mg Tablet contains Olmesartan Medoxomil, an angiotensin II receptor blocker (ARB), used to lower high blood pressure. It helps prevent strokes, heart attacks, and kidney problems.
Uses & Indications
Dosage
Adults
Recommended starting dose is 10 mg once daily. May be increased to 20 mg once daily, and if needed, to a maximum of 40 mg once daily.
Elderly
No initial dosage adjustment is typically required, but caution is advised.
Renal_impairment
Patients with moderate to severe renal impairment (creatinine clearance <40 mL/min) should start with 10 mg once daily. Max 20 mg once daily.
How to Take
Take orally once daily, with or without food, at approximately the same time each day.
Mechanism of Action
Olmesartan works by selectively blocking the binding of angiotensin II to the AT1 receptor in various tissues, including vascular smooth muscle and the adrenal gland. This prevents the vasoconstricting and aldosterone-secreting effects of angiotensin II, leading to vasodilation and reduced blood pressure.
Pharmacokinetics
Onset
Within 2 weeks, full effect in 4-8 weeks.
Excretion
Renal and hepatic (biliary excretion). Approximately 35-50% renally excreted, remainder via bile.
Half life
13 hours.
Absorption
Bioavailability is about 26%. Peak plasma concentration reached in 1-2 hours.
Metabolism
Olmesartan medoxomil is completely hydrolyzed to active olmesartan in the GI tract. No further metabolism.
Side Effects
Contraindications
- Hypersensitivity to Olmesartan Medoxomil or any component of the formulation
- Pregnancy (especially second and third trimesters)
- Concomitant use with aliskiren in patients with diabetes or renal impairment
Drug Interactions
NSAIDs
Reduced antihypertensive effect, potential for worsening renal function.
Lithium
Increased serum lithium concentrations and toxicity.
Diuretics
Increased risk of hypotension and renal impairment.
Potassium-sparing diuretics/Potassium supplements
Increased serum potassium levels (hyperkalemia).
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
The most likely manifestation of overdose is hypotension and tachycardia; bradycardia could occur. Treatment is symptomatic and supportive, including close monitoring of vital signs and administration of intravenous fluids if hypotension occurs. Olmesartan is not dialyzable.
Pregnancy & Lactation
Contraindicated in pregnancy due to risk of fetal injury and death, especially in the second and third trimesters. Not recommended during lactation as it may pass into breast milk.
