Description
Overview of the medicine
Olmetor 40 mg tablet contains Olmesartan Medoxomil, an angiotensin II receptor blocker used to treat high blood pressure (hypertension) and to reduce the risk of cardiovascular events. It works by relaxing blood vessels, allowing blood to flow more easily.
Uses & Indications
Dosage
Adults
Initial dose 20 mg once daily, adjusted to maximum 40 mg once daily based on blood pressure response. Can be taken with or without food.
Elderly
No initial dosage adjustment required for elderly patients, but careful monitoring is advised.
Renal_impairment
For mild to moderate renal impairment, no dosage adjustment is usually necessary. For severe impairment (creatinine clearance <20 mL/min), a starting dose of 10 mg once daily is recommended, with a maximum of 20 mg once daily.
How to Take
Take the tablet orally once daily, with or without food, at approximately the same time each day.
Mechanism of Action
Olmesartan Medoxomil is a prodrug that is hydrolyzed to olmesartan during absorption from the gastrointestinal tract. Olmesartan selectively blocks the binding of angiotensin II to the AT1 receptor in vascular smooth muscle and other tissues. This inhibition prevents the vasoconstrictive and aldosterone-secreting effects of angiotensin II, leading to vasodilation and reduced blood pressure.

Onset
Within 2 weeks for full effect, initial effect within 1 week.
Excretion
Approximately 35-50% excreted via urine and the remainder via feces/bile.
Half life
Approximately 13 hours.
Absorption
Rapidly and completely absorbed from the gastrointestinal tract and rapidly converted to olmesartan. Bioavailability is approximately 26%.
Metabolism
Olmesartan is not metabolized by the cytochrome P450 system. It is extensively metabolized in the gut wall during absorption to its active form, olmesartan.
Side Effects
Contraindications
- Hypersensitivity to olmesartan medoxomil or any component of the product.
- Pregnancy (second and third trimesters).
- Concomitant use with aliskiren in patients with diabetes or renal impairment (GFR <60 mL/min/1.73 m²).
- Biliary obstruction.
Drug Interactions
Lithium
Increased serum lithium concentrations and toxicity. Monitor lithium levels.
Diuretics
Increased risk of hypotension, especially with high doses of diuretics.
ACE Inhibitors, Aliskiren
Increased risk of hypotension, hyperkalemia, and renal impairment (particularly in patients with diabetes or renal impairment). Avoid concomitant use.
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
May reduce the antihypertensive effect of olmesartan. Increased risk of renal impairment, especially in elderly, volume-depleted, or those with impaired renal function.
Potassium-sparing diuretics, potassium supplements, salt substitutes containing potassium
Increased risk of hyperkalemia. Monitor serum potassium levels.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include hypotension and tachycardia; bradycardia may occur. Treatment is supportive and symptomatic. If ingestion is recent, gastric lavage may be considered. Administer intravenous fluids to support blood pressure. Olmesartan is not removed by hemodialysis.
Pregnancy & Lactation
Category D in pregnancy (second and third trimesters) due to risk of fetal harm. Not recommended during breastfeeding as it is unknown if olmesartan is excreted in human milk. Discuss risks and benefits with a healthcare provider.
