Description
Overview of the medicine
Omeprazole is a proton pump inhibitor used to treat conditions caused by too much stomach acid. It helps reduce stomach acid production, relieving symptoms like heartburn and acid reflux.
Uses & Indications
Dosage
Adults
For GERD: 20 mg once daily for 4-8 weeks. For duodenal ulcer: 20 mg once daily for 2-4 weeks. For gastric ulcer: 20 mg once daily for 4-8 weeks. For H. pylori eradication: 20 mg twice daily with appropriate antibiotics for 7-14 days. For Zollinger-Ellison Syndrome: Initial dose 60 mg once daily, adjusted as needed.
Elderly
No dosage adjustment generally required, but caution is advised in severe hepatic impairment.
Renal_impairment
No dosage adjustment required.
How to Take
Swallow the capsule whole with water before a meal, preferably in the morning. Do not chew or crush.
Mechanism of Action
Omeprazole works by irreversibly binding to and inhibiting the H+/K+-ATPase (proton pump) in the parietal cells of the stomach, thereby reducing gastric acid secretion.

Onset
Within 1 hour
Excretion
Excreted primarily in the urine (about 80%) and feces (about 20%) as metabolites.
Half life
0.5-1 hour, but the duration of acid suppression is much longer due to irreversible binding.
Absorption
Rapidly absorbed from the gastrointestinal tract, bioavailability is about 30-40% for the first dose, increasing to 60-70% after repeated doses.
Metabolism
Extensively metabolized in the liver, primarily by CYP2C19 and CYP3A4.

Contraindications
- Hypersensitivity to omeprazole or other benzimidazoles
- Concomitant use with nelfinavir
Drug Interactions
Warfarin
Increased INR/PT reported, monitor closely.
Clopidogrel
Omeprazole may reduce the antiplatelet effect of clopidogrel by inhibiting CYP2C19. Avoid concomitant use if possible.
Methotrexate
Increased methotrexate levels, especially with high doses.
Diazepam, Phenytoin, Citalopram
May increase plasma levels due to CYP2C19 inhibition.
Storage
Store below 30°C, in a dry place, away from direct sunlight.
Overdose
Symptoms may include nausea, vomiting, dizziness, abdominal pain, diarrhea, and headache. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Use during pregnancy only if clearly needed and the potential benefits outweigh the risks. Excreted in breast milk, use with caution during lactation.
