Description
Overview of the medicine
Omecap 40 mg Injection contains Omeprazole, which is a proton pump inhibitor (PPI). It works by reducing the amount of acid produced in the stomach. It is used to treat conditions such as peptic ulcers, gastroesophageal reflux disease (GERD), and Zollinger-Ellison syndrome, especially when oral administration is not possible.
Uses & Indications
Dosage
Adults
Usually 40 mg IV once daily. In severe cases or Zollinger-Ellison syndrome, initial dose may be 80 mg IV followed by 40 mg every 12 hours. Dosage may vary depending on indication and patient's condition.
Elderly
No specific dose adjustment is generally required, but close monitoring is advisable.
Renal_impairment
No dose adjustment is necessary for patients with impaired renal function.
How to Take
Omecap 40 mg Injection is administered as an intravenous infusion. The powder must be reconstituted with the provided solvent and then infused slowly over 20-30 minutes. It should be administered by a healthcare professional.
Mechanism of Action
Omeprazole is a substituted benzimidazole that suppresses gastric acid secretion by specific inhibition of the H+/K+-ATPase enzyme system (the 'proton pump') in the gastric parietal cells. It irreversibly blocks the final step in acid production.
Pharmacokinetics
Onset
Acid secretion inhibition occurs within 30-60 minutes after intravenous administration.
Excretion
Mainly excreted in the urine (as metabolites) and to a lesser extent in the feces.
Half life
Plasma half-life is approximately 0.5-1 hour, but the duration of acid suppression is much longer due to irreversible binding to the proton pump.
Absorption
Rapidly absorbed after intravenous administration, with complete bioavailability.
Metabolism
Extensively metabolized in the liver, primarily by CYP2C19 and CYP3A4 enzymes.
Side Effects
Contraindications
- •Hypersensitivity to omeprazole, substituted benzimidazoles, or any component of the formulation.
- •Concomitant use with nelfinavir (an antiretroviral drug).
Drug Interactions
Warfarin
Increased INR and risk of bleeding; monitor coagulation parameters.
Clopidogrel
Decreased antiplatelet effect of clopidogrel due to inhibition of CYP2C19.
Methotrexate
Increased and prolonged serum levels of methotrexate, possibly leading to toxicity.
Diazepam, Phenytoin, Cilostazol
Delayed elimination and increased plasma concentrations of these drugs.
Drugs requiring acidic environment for absorption (e.g., Ketoconazole, Iron salts)
Reduced absorption due to gastric acid suppression.
Storage
Store below 30°C. Protect from light and moisture. Reconstituted solution should be used within a specific time (e.g., 6 hours) and stored below 25°C, as per manufacturer's instructions.
Overdose
In case of overdose, symptoms are generally mild and reversible. There is no specific antidote. Treatment should be symptomatic and supportive.
Pregnancy & Lactation
Omeprazole is categorized as Pregnancy Category C. It should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Omeprazole is excreted in breast milk; therefore, caution should be exercised when administered to a nursing mother.
