Onasia 4 mg Oral Solution is an antiemetic used to prevent nausea and vomiting caused by chemotherapy, radiation therapy, and surgery. It works by blocking serotonin, a natural substance in the body that can cause nausea and vomiting.
Uses & Indications
Dosage
Adults
Typically 8 mg orally 30 minutes before chemotherapy, then 8 mg every 8 hours for up to 5 days. For other indications, dose varies.
Elderly
No dosage adjustment generally required. Patients over 75 years may require adjustment based on individual response.
Renal_impairment
No dosage adjustment required for mild to moderate renal impairment.
How to Take
For oral solution, measure the prescribed dose using the provided dosing syringe or spoon. It can be taken with or without food.
Mechanism of Action
Onasia selectively antagonizes the 5-HT3 receptor, located peripherally on vagal nerve terminals and centrally in the chemoreceptor trigger zone. This action blocks serotonin's ability to stimulate the vomiting reflex.

Excretion
Approximately 5-10% excreted unchanged in urine; remainder excreted as metabolites in urine and feces.

Absorption
Rapidly and completely absorbed from the gastrointestinal tract; peak plasma concentrations reached within 1.5 hours.
Metabolism
Extensively metabolized in the liver by hydroxylation, followed by glucuronide or sulfate conjugation. CYP3A4, CYP2D6, and CYP1A2 enzymes are involved.

Contraindications
- Hypersensitivity to Onasia or any component of the formulation
- Concomitant use with apomorphine
Drug Interactions
Tramadol
May reduce the analgesic effect of tramadol.
Apomorphine
Concomitant use is contraindicated due to reports of profound hypotension and loss of consciousness.
Phenytoin, Carbamazepine, Rifampicin
May decrease Onasia plasma concentrations by inducing hepatic enzymes.
