Overview of the medicine
Onasia 8 mg Tablet contains Ondansetron, a selective 5-HT3 receptor antagonist used to prevent nausea and vomiting caused by chemotherapy, radiation therapy, and surgery.
Uses & Indications
Dosage
Adults
Chemotherapy-induced nausea/vomiting: Typically 8 mg orally 30 minutes before chemotherapy, then 8 mg every 8 hours for 1-2 days. Postoperative nausea/vomiting: 8 mg orally one hour before anesthesia, or 16 mg orally one hour before anesthesia.
Elderly
No dosage adjustment generally required, but close monitoring for QT prolongation is advised.
Renal_impairment
No dosage adjustment recommended for patients with impaired renal function.
How to Take
Take the tablet whole with or without food. For prevention of chemotherapy-induced nausea/vomiting, take 30 minutes before chemotherapy. For postoperative nausea/vomiting, take one hour before anesthesia.
Mechanism of Action
Ondansetron selectively blocks 5-hydroxytryptamine3 (5-HT3) receptors located both peripherally on vagal nerve terminals and centrally in the chemoreceptor trigger zone (CTZ) of the area postrema. This action prevents both central and peripheral initiation of the vomiting reflex.

Onset
30 minutes to 2 hours (oral)
Excretion
Primarily renal (about 5% as unchanged drug), some fecal.
Half life
Approximately 3-5 hours (adults); up to 20 hours (elderly)
Absorption
Rapidly and completely absorbed from the GI tract after oral administration, but undergoes some first-pass metabolism.
Metabolism
Extensive hepatic metabolism, primarily by CYP3A4, CYP2D6, and CYP1A2.
Side Effects
Contraindications
- Known hypersensitivity to ondansetron or any component of the formulation.
- Concomitant use with apomorphine due to risk of profound hypotension and loss of consciousness.
Drug Interactions
Tramadol
May reduce the analgesic effect of tramadol.
Apomorphine
Concomitant use is contraindicated due to severe hypotension.
QT prolonging drugs
Increased risk of QT prolongation and arrhythmias (e.g., antiarrhythmics, antipsychotics, some antibiotics).
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include transient blindness, severe constipation, hypotension, and vasovagal episodes with transient second-degree AV block. Management is supportive and symptomatic.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use only if clearly needed. It is not known whether ondansetron is excreted in human milk; caution is advised when administered to a nursing woman.
