Overview of the medicine
Onclav 125 mg Suspension is an antibiotic medication containing Amoxicillin and Clavulanic Acid. It is used to treat a wide range of bacterial infections by killing bacteria and preventing them from becoming resistant to Amoxicillin.
Uses & Indications
Dosage
Adults
Adult dosage typically uses higher strength formulations. If this suspension is used, the equivalent dose would be higher (e.g., 250-500mg Amoxicillin equivalent).
Children
For children 3 months to 12 years (based on Amoxicillin component): 20 mg/kg/day for mild to moderate infections in 3 divided doses. For severe infections: 40 mg/kg/day in 3 divided doses. (e.g., 5ml of 125mg/31.25mg suspension three times daily for a 10kg child).
Renal_impairment
Dose adjustment required based on creatinine clearance for both children and adults. Consult prescribing information for specific guidelines.
How to Take
Administer orally, usually at the start of a meal to minimize potential gastrointestinal intolerance. Shake the bottle well before each use. Use a measuring spoon or oral syringe for accurate dosing.
Mechanism of Action
Amoxicillin is a penicillin-class antibiotic that inhibits bacterial cell wall synthesis. Clavulanic Acid is a beta-lactamase inhibitor that protects Amoxicillin from degradation by beta-lactamase enzymes produced by resistant bacteria, thus extending Amoxicillin's spectrum of activity.

Excretion
Primarily excreted unchanged by the kidneys (glomerular filtration and tubular secretion) for Amoxicillin. Clavulanic acid is also excreted by the kidneys and via non-renal mechanisms.

Approximately 1 to 1.5 hours for both components.
Absorption
Both Amoxicillin and Clavulanic Acid are well absorbed after oral administration. Absorption is not significantly affected by food.
Metabolism
Amoxicillin is partially metabolized to inactive penicilloic acid. Clavulanic acid is extensively metabolized.
Side Effects
Contraindications
- •History of hypersensitivity to Amoxicillin, Clavulanic Acid, or any other penicillin or beta-lactam antibiotic.
- •History of severe hepatic dysfunction associated with Amoxicillin/Clavulanic Acid.
Drug Interactions
Probenecid
Increases Amoxicillin levels by decreasing renal tubular secretion.
Allopurinol
Increased risk of rash with Amoxicillin.
Methotrexate
Increases methotrexate toxicity due to reduced renal clearance.
Oral contraceptives
May reduce efficacy of oral contraceptives (rarely significant).
Oral anticoagulants (e.g., Warfarin)
May prolong prothrombin time; monitor INR closely.
Storage
Store dry powder below 25°C in a dry place. Store reconstituted suspension in a refrigerator (2-8°C). Do not freeze. Keep out of reach of children.
Overdose
Symptoms of overdose may include gastrointestinal symptoms (nausea, vomiting, diarrhea) and disturbances of the fluid and electrolyte balance. In severe cases, crystalluria leading to renal failure has been observed. Treatment is symptomatic and supportive; hemodialysis may be used to remove the drug from circulation.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus. Use in pregnancy only if clearly needed. Both Amoxicillin and Clavulanic Acid are excreted into breast milk; use with caution during lactation.
