Description
Overview of the medicine
Onclav 500 mg Tablet is a combination antibiotic used to treat a wide range of bacterial infections. It contains Amoxicillin, a penicillin antibiotic, and Clavulanic Acid, a beta-lactamase inhibitor, which works by protecting Amoxicillin from degradation by bacterial enzymes, thereby enhancing its antibacterial spectrum.
Uses & Indications
Dosage
Adults
The usual adult dose is 500 mg/125 mg every 8-12 hours, depending on the severity and type of infection. For severe infections, 875 mg/125 mg every 12 hours may be used.
Elderly
No specific dose adjustment is required in elderly patients with normal renal function. Renal function should be monitored.
Renal_impairment
Dose adjustment is necessary for patients with renal impairment based on creatinine clearance. For CrCl 10-30 mL/min, 500 mg/125 mg every 12 hours. For CrCl <10 mL/min, 500 mg/125 mg every 24 hours.
How to Take
Take the tablet whole with a glass of water. It can be taken with or without food, but taking it at the start of a meal may reduce gastrointestinal discomfort. Do not chew or crush the tablet.
Mechanism of Action
Amoxicillin acts by inhibiting bacterial cell wall synthesis, leading to cell lysis and bacterial death. Clavulanic Acid is a beta-lactamase inhibitor that binds irreversibly to beta-lactamase enzymes produced by some bacteria, preventing the inactivation of Amoxicillin and allowing it to exert its bactericidal effect.

Onset
Antibacterial effects typically begin within 1-2 hours after administration.
Excretion
Both components are primarily excreted by the kidneys, with Amoxicillin being excreted largely unchanged and Clavulanic Acid as metabolites.
Half life
The elimination half-life for Amoxicillin is approximately 1-1.5 hours, and for Clavulanic Acid, it is about 1 hour.
Absorption
Both Amoxicillin and Clavulanic Acid are well absorbed from the gastrointestinal tract after oral administration. Peak plasma concentrations are achieved in approximately 1-2 hours.
Metabolism
Amoxicillin is minimally metabolized, primarily excreted unchanged. Clavulanic Acid undergoes extensive metabolism.
Side Effects
Contraindications
- •History of hypersensitivity to penicillins, cephalosporins, or other beta-lactam antibiotics.
- •History of amoxicillin/clavulanate-associated jaundice or hepatic dysfunction.
Drug Interactions
Probenecid
Increases and prolongs Amoxicillin blood levels.
Allopurinol
Increased incidence of skin rash, particularly in patients with hyperuricemia.
Methotrexate
Increased methotrexate toxicity due to reduced renal tubular secretion.
Oral contraceptives
May reduce the efficacy of oral contraceptives (though clinical significance is debated).
Anticoagulants (e.g., Warfarin)
May prolong prothrombin time and increase INR, requiring close monitoring.
Storage
Store in a cool, dry place below 30°C. Protect from moisture and light. Keep out of reach of children.
Overdose
Overdose may lead to gastrointestinal symptoms such as abdominal pain, vomiting, and diarrhea, as well as electrolyte imbalance and crystalluria. In cases of severe overdose, renal impairment may occur. Treatment is symptomatic and supportive. Hemodialysis can remove both Amoxicillin and Clavulanic Acid from circulation.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but adequate and well-controlled studies in pregnant women are lacking. Use during pregnancy only if clearly needed. Both Amoxicillin and Clavulanic Acid are excreted in breast milk. Caution should be exercised when administered to a nursing mother due to potential for sensitization or gastrointestinal effects in the infant.
