Overview of the medicine
Ondansetron is an antiemetic used to prevent nausea and vomiting caused by chemotherapy, radiation therapy, and surgery.
Uses & Indications
Dosage
Adults
Chemotherapy-induced nausea and vomiting: 8 mg oral, 1-2 hours before chemotherapy, followed by 8 mg every 8-12 hours for up to 5 days. Post-operative nausea and vomiting: 4 mg oral, 1 hour before anesthesia, or 8 mg oral up to 8 hours after surgery.
Elderly
No specific dose adjustment is generally required for elderly patients, but caution is advised in very frail individuals.
Renal_impairment
No specific dose adjustment is required.
How to Take
May be taken with or without food. Measure the dose carefully using the provided measuring device.
Mechanism of Action
Ondansetron selectively antagonizes serotonin 5-HT3 receptors located both peripherally on vagal nerve terminals and centrally in the chemoreceptor trigger zone.
Pharmacokinetics
Onset
Within 30 minutes to 2 hours.
Excretion
About 5% of the dose is excreted unchanged in the urine; the remainder is excreted as metabolites in urine and feces.
Half life
Approximately 3-6 hours in adults.
Absorption
Well absorbed from the gastrointestinal tract; bioavailability is about 60%.
Metabolism
Extensively metabolized in the liver by cytochrome P450 enzymes (CYP1A2, CYP2D6, CYP3A4).
Side Effects
Contraindications
- Hypersensitivity to ondansetron or any component of the formulation.
- Concomitant use with apomorphine.
Drug Interactions
Tramadol
May reduce the analgesic effect of tramadol.
Apomorphine
Concurrent use may cause profound hypotension and loss of consciousness.
QT prolonging drugs
Increased risk of QT interval prolongation.
Phenytoin, Carbamazepine, Rifampicin
May decrease ondansetron levels.

Store below 30°C in a dry place, away from light and moisture. Do not freeze.
Overdose
Symptoms may include visual disturbances, severe constipation, hypotension, and vagal episode with transient AV block. Treatment is supportive.
