Overview of the medicine
Opmax 20 mg Capsule is a proton pump inhibitor used to treat conditions where the stomach produces too much acid, such as ulcers, GERD, and erosive esophagitis.
Uses & Indications
Dosage
Adults
For most conditions, 20 mg once daily for 4-8 weeks. For severe cases or Zollinger-Ellison syndrome, higher doses may be required (up to 60 mg/day). For H. pylori eradication, 20 mg twice daily with antibiotics.
Elderly
No specific dose adjustment is generally required, but caution is advised due to potential for decreased hepatic function.
Renal_impairment
No dose adjustment needed for renal impairment.
How to Take
Take orally, preferably in the morning before breakfast. Swallow the capsule whole with water. Do not chew or crush the capsule.
Mechanism of Action
Omeprazole works by irreversibly blocking the hydrogen/potassium adenosine triphosphatase (H+/K+ ATPase) enzyme system, also known as the proton pump, in the gastric parietal cells. This action significantly reduces both basal and stimulated gastric acid secretion.

Within 1 hour
Excretion
Mainly excreted in urine (about 80%) and feces (about 20%) as metabolites.
Half life
0.5 to 1 hour (terminal elimination half-life is longer due to irreversible binding to proton pump)
Absorption
Rapidly absorbed from the gastrointestinal tract, peak plasma concentrations reached within 0.5-3.5 hours. Bioavailability is about 30-40%.
Metabolism
Extensively metabolized in the liver, primarily by CYP2C19 and CYP3A4 enzymes.
Side Effects
Contraindications
- •Hypersensitivity to omeprazole, substituted benzimidazoles, or any component of the formulation
- •Concomitant use with nelfinavir
Drug Interactions
Warfarin
Increased INR/prothrombin time, monitor carefully.
Clopidogrel
Decreased antiplatelet effect, consider alternative PPIs.
Methotrexate
Increased methotrexate levels, especially with high-dose therapy.
Nelfinavir, Atazanavir
Decreased plasma levels of antivirals, avoid concomitant use.
Diazepam, Phenytoin, Citalopram
Increased plasma levels due to CYP2C19 inhibition.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms usually mild: nausea, vomiting, abdominal pain, diarrhea, dizziness, headache. Treatment is symptomatic and supportive. No specific antidote.
Pregnancy & Lactation
Generally considered safe in pregnancy, but use only if clearly needed after consulting a doctor. Excreted in breast milk, use with caution during lactation and monitor the infant.
