Overview of the medicine
Opmax 40 mg Capsule contains Omeprazole, a proton pump inhibitor used to reduce stomach acid production, treating conditions like ulcers, GERD, and Zollinger-Ellison syndrome.
Uses & Indications
Dosage
Adults
Typical dosage for GERD and ulcers is 20-40 mg once daily for 4-8 weeks. For Zollinger-Ellison syndrome, dosages may be higher, individualized to patient response.
Elderly
No specific dose adjustment is usually required, but caution is advised due to potential age-related decrease in renal or hepatic function.
Renal_impairment
No dosage adjustment is generally needed for renal impairment.
How to Take
Take the capsule whole with water at least 30-60 minutes before a meal, preferably in the morning. Do not chew or crush.
Mechanism of Action
Omeprazole selectively inhibits the H+/K+-ATPase enzyme system (proton pump) in the gastric parietal cells, thereby blocking the final step of acid production.
Pharmacokinetics
Onset
Acid secretion inhibition begins within 1 hour, maximum effect within 2 hours.
Excretion
Excreted mainly in the urine (about 77%) and feces (about 23%).
Half life
Plasma half-life is approximately 0.5 to 1 hour, but the acid-inhibiting effect lasts much longer (up to 72 hours) due to irreversible binding to the proton pump.
Absorption
Rapidly absorbed after oral administration, peak plasma concentrations reached within 0.5 to 3.5 hours.
Metabolism
Extensively metabolized in the liver, primarily by CYP2C19 and CYP3A4.
Side Effects
Contraindications
- •Hypersensitivity to omeprazole or any component of the formulation
- •Concomitant use with nelfinavir
Drug Interactions
Digoxin
Increased absorption of digoxin.
Warfarin
Increased INR/prothrombin time; monitor closely.
Clopidogrel
Omeprazole reduces the antiplatelet effect of clopidogrel.
Methotrexate
Increased plasma levels of methotrexate.
Diazepam, Phenytoin
Prolonged elimination of these drugs.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include nausea, vomiting, dizziness, abdominal pain, diarrhea, headache. Treatment is supportive and symptomatic.
Pregnancy & Lactation
Use in pregnancy only if clearly needed. Excreted in breast milk; caution advised during lactation.
