Description
Overview of the medicine
Optaphenicol 1% Eye Ointment is a broad-spectrum antibiotic ophthalmic preparation used to treat bacterial infections of the eye. It is effective against a wide range of gram-positive and gram-negative bacteria.
Uses & Indications
Dosage
Adults
Apply a small ribbon (approx. 1 cm) into the conjunctival sac 3-4 times daily.
Elderly
Same as adult dosage.
Renal_impairment
No dosage adjustment needed for topical ophthalmic use.
How to Take
Wash hands before use. Gently pull down the lower eyelid to create a small pocket. Squeeze a small amount of ointment into the pocket. Close the eye gently for 1-2 minutes. Avoid touching the tip of the tube to the eye or any other surface.
Mechanism of Action
Chloramphenicol inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit, thereby preventing peptide bond formation. This leads to bacteriostatic activity against susceptible organisms.
Pharmacokinetics
Onset
Within hours of application
Excretion
Mainly via renal excretion for any systemically absorbed drug, but local drug is cleared from ocular surface.
Half life
Not well-defined for topical ophthalmic use due to minimal systemic absorption; estimated short duration in ocular tissues.
Absorption
Minimal systemic absorption occurs after topical ophthalmic administration. Local absorption into ocular tissues is sufficient for therapeutic effect.
Metabolism
Primarily hepatic for systemically absorbed drug, but minimal metabolism expected with topical ophthalmic use.
Side Effects
Contraindications
- Hypersensitivity to chloramphenicol or any component of the ointment
- History of bone marrow depression with chloramphenicol use
- Minor infections where alternative agents are suitable
Drug Interactions
General
No significant systemic drug interactions expected due to minimal topical absorption. However, concurrent use of other ophthalmic preparations should be separated by at least 5 minutes.
Storage
Store below 25°C. Do not freeze. Keep out of reach of children. Discard any remaining ointment 28 days after opening.
Overdose
Overdose from topical ophthalmic application is unlikely. If excessive amount is applied, wipe away the excess. If accidentally ingested, seek medical advice.
Pregnancy & Lactation
Use with caution during pregnancy and lactation. Consult a physician. Minimal systemic absorption makes risk low, but potential for adverse effects on fetus/infant cannot be entirely ruled out.
