Description
Overview of the medicine
Optison-N eye ointment is a combination medication containing Dexamethasone, a potent corticosteroid, and Neomycin Sulfate, an aminoglycoside antibiotic. It is used to treat inflammatory conditions of the eye that are also complicated by bacterial infections susceptible to Neomycin.
Uses & Indications
Dosage
Adults
Apply a small amount (approximately 1/2 inch ribbon) into the conjunctival sac 3-4 times daily, or as directed by a physician. In severe cases, dosage may be increased to every 2-3 hours initially.
Elderly
No specific dose adjustment required for elderly patients, use adult dosage.
Renal_impairment
No specific dose adjustment required as systemic absorption is minimal.
How to Take
For ophthalmic use only. Wash hands thoroughly before application. Pull down the lower eyelid, apply a small ribbon of ointment into the conjunctival sac, and close the eye for a short period to allow spreading. Do not touch the tip of the tube to the eye or any other surface.
Mechanism of Action
Dexamethasone exerts its action by reducing inflammation, swelling, and itching. It inhibits the release of inflammatory mediators and modifies the body's immune response. Neomycin Sulfate is a broad-spectrum antibiotic that works by inhibiting bacterial protein synthesis, leading to the death of susceptible bacteria.
Pharmacokinetics
Onset
Anti-inflammatory effects of Dexamethasone typically within hours; antibacterial effects of Neomycin onset depends on bacterial load but usually within 1-2 days.
Excretion
Systemically absorbed Dexamethasone is excreted mainly via urine as metabolites. Neomycin is excreted largely unchanged via urine if absorbed systemically.
Half life
Systemic half-life of Dexamethasone is about 3-4 hours if absorbed; Neomycin systemic half-life is 2-3 hours. Local half-life in ocular tissues is not well-defined.
Absorption
Minimal systemic absorption after topical ocular administration. Local absorption into ocular tissues.
Metabolism
Systemically absorbed Dexamethasone is metabolized in the liver. Neomycin is minimally metabolized.
Side Effects
Contraindications
- Hypersensitivity to Dexamethasone, Neomycin, or any component of the formulation.
- Viral diseases of the cornea and conjunctiva (e.g., epithelial herpes simplex keratitis, vaccinia, varicella).
- Mycobacterial infection of the eye.
- Fungal diseases of ocular structures.
- Untreated purulent infection of the eye.
Drug Interactions
Systemic Aminoglycosides
Although systemic absorption is minimal, prolonged use of Neomycin-containing ophthalmic products concurrently with systemic aminoglycosides may increase the risk of ototoxicity or nephrotoxicity, especially in patients with pre-existing renal impairment.
Other Ophthalmic Preparations
If using multiple ophthalmic medications, allow at least 5-10 minutes between applications. Ointments should be applied last.
Storage
Store below 30°C. Do not freeze. Keep away from light and moisture. Keep out of reach of children.
Overdose
An acute overdose of ophthalmic preparation is unlikely to lead to life-threatening situations. If an excessive amount is applied, wipe off the excess. In case of accidental ingestion, seek medical advice.
Pregnancy & Lactation
Pregnancy: Category C. Use only if the potential benefit justifies the potential risk to the fetus. Lactation: It is not known whether topical ophthalmic administration can result in sufficient systemic absorption to produce detectable quantities in human milk. Exercise caution when administered to a nursing mother.

Clinical Trials
Clinical efficacy and safety established through studies for corticosteroid-antibiotic ophthalmic combinations. Specific trial data for Optison-N might be proprietary.
Lab Monitoring
- Regular monitoring of intraocular pressure (IOP) is advisable during prolonged use (more than 10 days) of corticosteroid-containing ophthalmic preparations.
- Periodic eye examinations to check for cataracts or other ocular complications.
Doctor Notes
- Emphasize limited duration of use to minimize corticosteroid-related side effects.
- Educate patients on proper application technique to prevent contamination.
- Monitor IOP if treatment exceeds 10 days.
Patient Guidelines
- Use only for the eye. Do not swallow.
- Do not wear contact lenses during treatment.
- Avoid contaminating the applicator tip.
Missed Dose Advice
If a dose is missed, apply it as soon as you remember. If it is almost time for the next dose, skip the missed dose and continue with the regular dosing schedule. Do not double the dose.
