Description
Overview of the medicine
Orabis 25 mg tablet contains Deflazacort, a corticosteroid used to treat various inflammatory and autoimmune conditions, including rheumatic disorders, asthma, allergies, and certain kidney diseases.
Uses & Indications
Dosage
Adults
Initial dose typically 6-90 mg daily, adjusted according to response. Duchenne muscular dystrophy: 0.9 mg/kg/day.
Elderly
No specific dose adjustment required, but use with caution due to potential comorbidities and polypharmacy.
Renal_impairment
No dose adjustment needed for renal impairment. Use with caution in severe renal impairment due to potential fluid retention.
How to Take
Take orally, preferably with food to minimize gastrointestinal upset. Do not stop abruptly; taper dose gradually.
Mechanism of Action
Deflazacort is a glucocorticoid that binds to specific intracellular glucocorticoid receptors. This complex then translocates into the nucleus, where it modulates gene expression, leading to anti-inflammatory and immunosuppressive effects. It reduces inflammation by inhibiting the release of inflammatory mediators and suppressing immune responses.
Pharmacokinetics
Onset
Anti-inflammatory effects typically begin within a few hours, full therapeutic effect may take several days.
Excretion
Mainly excreted in urine (about 70%) and feces (about 30%) as metabolites.
Half life
The active metabolite has a plasma half-life of approximately 1.5-2 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract, converted to its active metabolite (21-desacetyl deflazacort). Peak plasma concentrations of the active metabolite are achieved within 1.5-2 hours.
Metabolism
Extensively metabolized in the liver to the active metabolite, 21-desacetyl deflazacort, which is then further metabolized.
Side Effects
Contraindications
- Systemic fungal infections
- Hypersensitivity to deflazacort or any excipients
- Live virus vaccines during corticosteroid therapy
Drug Interactions
NSAIDs
Increased risk of gastrointestinal ulceration.
Anticoagulants
May alter anticoagulant effects; monitor INR.
CYP3A4 inducers (e.g., rifampicin)
May decrease deflazacort levels, reducing efficacy.
CYP3A4 inhibitors (e.g., ketoconazole)
May increase deflazacort levels, requiring dose adjustment.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Acute overdose is rare. Symptoms may include exaggeration of known side effects. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Pregnancy: Use only if the potential benefit justifies the potential risk to the fetus. Lactation: Excreted in breast milk; caution advised, weigh risks vs benefits.
