Overview of the medicine
Orbidex-T Eye Drop is a combination medication containing an antibiotic (Tobramycin) and a corticosteroid (Dexamethasone) used to treat inflammatory eye conditions where there is a risk of bacterial infection.
Uses & Indications
Dosage
Adults
Instill one or two drops into the conjunctival sac of the affected eye(s) every four to six hours. During the initial 24 to 48 hours, the dosage may be increased to one or two drops every two hours. The frequency should be gradually decreased as the clinical signs improve. Treatment should not exceed 14 days.
Elderly
No specific dosage adjustment is required for elderly patients.
Renal_impairment
No specific dosage adjustment is required due to minimal systemic absorption.
How to Take
For ophthalmic use only. Shake well before use. Do not touch the dropper tip to any surface to avoid contamination. After instillation, gently close the eye and apply pressure to the tear duct for a minute to reduce systemic absorption.
Mechanism of Action
Tobramycin is an aminoglycoside antibiotic that inhibits bacterial protein synthesis, leading to bacterial cell death. Dexamethasone is a potent corticosteroid that suppresses inflammation by inhibiting phospholipase A2 and various inflammatory mediators.

Within minutes to hours for symptomatic relief; full antibacterial effect within days.
Excretion
Mainly through kidneys for systemically absorbed portion, but topical absorption is very low.
Half life
Not extensively studied for topical ophthalmic application, but systemic half-life of tobramycin is 2-3 hours and dexamethasone 3-4 hours.
Absorption
Minimal systemic absorption after topical ophthalmic administration. Concentrations in ocular tissues are therapeutic.

Minimal metabolism for topical ophthalmic use.
Side Effects
Contraindications
- •Hypersensitivity to Tobramycin, Dexamethasone, or any component of the formulation.
- •Epithelial herpes simplex keratitis (dendritic keratitis), vaccinia, varicella, and other viral diseases of the cornea and conjunctiva.
- •Mycobacterial infection of the eye.
- •Fungal diseases of ocular structures.
Drug Interactions
Systemic aminoglycosides
Risk of increased systemic toxicity if significant systemic absorption occurs (rare with topical ophthalmic use).
Other ophthalmic preparations
If more than one topical ophthalmic medicine is being used, they should be administered at least 5 minutes apart.
Storage
Store below 30°C in a dry place, protected from light. Do not freeze. Keep the bottle tightly closed when not in use.
Overdose
Ocular overdose is unlikely to cause systemic toxicity due to limited absorption. Excessive topical application may be flushed from the eye with lukewarm water. Manage symptomatically if irritation occurs.
Pregnancy & Lactation
Use during pregnancy only if the potential benefit outweighs the potential risk to the fetus. Dexamethasone may appear in breast milk. Consult a doctor before use during pregnancy or lactation.
