Description
Overview of the medicine
Orbidex-T is an ophthalmic ointment containing Tobramycin, an aminoglycoside antibiotic, and Dexamethasone, a potent corticosteroid. It is used to treat bacterial infections of the eye and surrounding tissues that are accompanied by inflammation.
Uses & Indications
Dosage
Adults
Apply a small ribbon (approximately 1-1.5 cm) into the conjunctival sac 3 to 4 times a day.
Elderly
No specific dosage adjustment required; use with caution due to potential for increased ocular pressure.
Renal_impairment
No specific dosage adjustment required as systemic absorption is minimal.
How to Take
For ophthalmic use only. Wash hands thoroughly before application. Pull down the lower eyelid to form a pocket, apply a small amount of ointment into the pocket, and then close the eye gently. Avoid touching the tip of the tube to the eye or any other surface.
Mechanism of Action
Tobramycin works by inhibiting bacterial protein synthesis, leading to bacterial cell death. Dexamethasone suppresses inflammatory responses by inhibiting the release of inflammatory mediators and altering immune responses.
Pharmacokinetics
Onset
Within hours for anti-inflammatory effect, within days for antimicrobial effect.
Excretion
Mainly through local elimination and tear turnover; minimal systemic excretion.
Half life
Not significant systemically; local effects persist for several hours.
Absorption
Minimal systemic absorption occurs when applied topically to the eye. Local absorption into ocular tissues is significant.
Metabolism
Minimal systemic metabolism. Primarily metabolized in ocular tissues to inactive forms.

Drug Interactions
Topical NSAIDs
May increase the risk of corneal complications if used concurrently with topical steroids.
Aminoglycosides (systemic)
Although systemic absorption is minimal, theoretical risk of increased systemic toxicity if used concurrently with systemic aminoglycosides.
Other topical corticosteroids
Concurrent use may increase the risk of elevated intraocular pressure.
Storage
Store at room temperature (below 30°C). Do not freeze. Keep out of reach of children.
Overdose
An acute overdose is unlikely to cause life-threatening problems. However, if excess ointment is applied, it can be flushed from the eye with lukewarm water. In case of accidental ingestion, seek medical advice.
Pregnancy & Lactation
Pregnancy Category C. Use only if the potential benefit justifies the potential risk to the fetus. Caution should be exercised when administered to a nursing mother. Consult a physician.
