Orgaflu 125 mg Suspension is an antiviral medication used for the treatment and prevention of influenza (flu) caused by influenza A and B viruses. It works by preventing the virus from spreading in the body.
Uses & Indications
Dosage
Adults
For treatment: 75 mg twice daily for 5 days. For prophylaxis: 75 mg once daily for 10 days. (Note: Specific dosing for 125 mg suspension may vary based on product instructions and physician's guidance. Administer appropriate volume to achieve desired dose).
Elderly
No dosage adjustment needed unless there is significant renal impairment. Use adult dosage with caution.
Renal_impairment
For CrCl 10-30 mL/min: Treatment - 30 mg once daily for 5 days; Prophylaxis - 30 mg every other day. Severe renal impairment (CrCl <10 mL/min) or end-stage renal disease: Not recommended unless potential benefit outweighs risk.

Mechanism of Action
Oseltamivir is a prodrug that is converted to its active metabolite, oseltamivir carboxylate (OC), primarily by esterases in the liver. OC selectively inhibits the neuraminidase enzyme of influenza viruses. This enzyme is crucial for the release of new viral particles from infected cells and for the spread of the virus within the respiratory tract.

Excretion
Oseltamivir carboxylate is primarily eliminated unchanged by renal excretion (glomerular filtration and tubular secretion). Less than 20% is eliminated via feces.

Absorption
Rapidly absorbed from the gastrointestinal tract and extensively converted to oseltamivir carboxylate (OC) by hepatic esterases. Oral bioavailability of OC is approximately 75%.

Side Effects
Contraindications

Drug Interactions
Probenecid

Live Attenuated Influenza Vaccine (LAIV)
Avoid use of oseltamivir within 48 hours before and 2 weeks after LAIV administration, as oseltamivir may inhibit LAIV replication and reduce its efficacy.
Storage
Store the dry powder at room temperature, below 30°C (86°F). Store the reconstituted suspension in a refrigerator between 2°C to 8°C (36°F to 46°F). Do not freeze. Discard any unused portion after 17 days.
Overdose
Experience with overdose is limited. Symptoms observed in overdose cases include nausea, vomiting, dizziness, and headache. In some cases, accidental overdose in children resulted in more severe adverse events, including seizures. Management should be supportive and symptomatic.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Low levels of oseltamivir are excreted into human breast milk; caution should be exercised when administered to a nursing woman.
