Description
Overview of the medicine
Oriham (Omeprazole) is a proton pump inhibitor (PPI) used to reduce stomach acid production. It is prescribed for conditions such as gastroesophageal reflux disease (GERD), stomach ulcers, duodenal ulcers, and Zollinger-Ellison syndrome.
Uses & Indications
Dosage
Adults
Typically 20 mg once daily for 4-8 weeks for GERD and ulcers. For Zollinger-Ellison syndrome, 60 mg once daily, adjusted as needed. H. pylori eradication usually involves 20 mg twice daily with antibiotics.
Elderly
No specific dose adjustment is generally required, but caution is advised due to potential age-related decrease in renal and hepatic function.
Renal_impairment
No dose adjustment is necessary for patients with impaired renal function.
How to Take
Take capsule whole with water, preferably in the morning before a meal. Do not chew or crush. If swallowing is difficult, the capsule may be opened and the contents mixed with a small amount of fruit juice or yogurt and swallowed immediately.
Mechanism of Action
Omeprazole works by irreversibly binding to and inhibiting the H+/K+-ATPase proton pump in the gastric parietal cells, thereby blocking the final step of acid secretion.
Pharmacokinetics
Onset
Onset of acid suppression within 1 hour, maximal effect within 2 hours.
Excretion
Mainly excreted in the urine (about 80%) and feces (about 20%) as metabolites.
Half life
0.5-1 hour (plasma elimination half-life), but the duration of acid suppression is much longer due to irreversible binding.
Absorption
Rapidly but variably absorbed from the gastrointestinal tract; bioavailability is about 30-40% but increases with repeated administration. Peak plasma concentrations are reached within 0.5-3.5 hours.
Metabolism
Extensively metabolized in the liver, primarily by CYP2C19 and CYP3A4 enzymes.
Side Effects
Contraindications
- Hypersensitivity to omeprazole, substituted benzimidazoles, or any component of the formulation.
- Concomitant use with nelfinavir.

Diazepam
Increased plasma concentrations of diazepam due to inhibition of hepatic metabolism.
Warfarin
Increased INR and prothrombin time; monitor closely.
Tacrolimus
Increased plasma concentrations of tacrolimus.
Clopidogrel
Reduced antiplatelet effect of clopidogrel due to inhibition of CYP2C19.
Methotrexate
Increased plasma levels of methotrexate, particularly at high doses.
Storage
Store in a cool, dry place, protected from light and moisture, below 30°C. Keep out of reach of children.

Symptoms of overdose are usually mild and reversible, including nausea, vomiting, dizziness, abdominal pain, diarrhea, and headache. Treatment is symptomatic and supportive.

Pregnancy Category C. Use only if potential benefit outweighs the potential risk to the fetus. It is excreted in human milk; use with caution in nursing mothers.
