Description
Overview of the medicine
Ornova 10mg Tablet contains Olanzapine, an atypical antipsychotic medication used to treat schizophrenia and bipolar disorder. It helps to restore the balance of certain natural substances in the brain.
Uses & Indications
Dosage
Adults
Schizophrenia: Initial 5-10 mg once daily, target dose 10 mg/day. Max 20 mg/day. Bipolar Mania: Initial 10-15 mg once daily, target dose 15 mg/day. Max 20 mg/day.
Elderly
Lower starting dose (e.g., 5 mg/day) and slower titration, especially if other risk factors are present. Closely monitor for orthostatic hypotension.
Renal_impairment
No routine dosage adjustment required, but caution advised due to limited experience in severe renal impairment. Careful monitoring is recommended.
How to Take
Take orally once daily, with or without food. Do not crush or chew the tablet. Swallow whole.
Mechanism of Action
Olanzapine is a potent antagonist of serotonin 5-HT2A and dopamine D1, D2, D4 receptors. It also binds to muscarinic M1-M5, adrenergic α1, and histamine H1 receptors. Its antipsychotic effect is thought to be mediated through a combination of dopamine and serotonin receptor antagonism.
Pharmacokinetics
Onset
Clinical response typically observed within days to weeks for acute symptoms; maintenance treatment requires continuous therapy.
Excretion
Primarily renal excretion (60% as metabolites); about 30% excreted in feces.
Half life
Approximately 21-54 hours (mean 33 hours).
Absorption
Well absorbed orally, reaching peak plasma concentrations in 5-8 hours. Food does not significantly affect the rate or extent of absorption.
Metabolism
Extensively metabolized in the liver primarily by direct glucuronidation and by cytochrome P450-mediated oxidation (CYP1A2 and CYP2D6).
Side Effects
Contraindications
- Hypersensitivity to olanzapine or any component of the formulation.
- Patients with known narrow-angle glaucoma.
Drug Interactions
Fluoxetine
May increase olanzapine concentration.
Antihypertensives
Increased risk of orthostatic hypotension.
Carbamazepine, Rifampin
May decrease olanzapine levels (CYP1A2 inducers).
Alcohol, CNS depressants
Enhanced sedative effects.
Fluvoxamine, Ciprofloxacin
May increase olanzapine levels (CYP1A2 inhibitors).
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include somnolence, slurred speech, tachycardia, extrapyramidal symptoms, and respiratory depression. Management is symptomatic and supportive, including maintaining an open airway, oxygenation, and ventilation. Activated charcoal may be useful.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit outweighs potential risk to the fetus. Neonates exposed to antipsychotics during the third trimester are at risk for extrapyramidal and/or withdrawal symptoms. Olanzapine is excreted in human milk; breastfeeding is not recommended.
