Description
Overview of the medicine
Osmosol is an osmotic diuretic primarily used to reduce intracranial pressure, treat cerebral edema, and lower intraocular pressure. It can also be used to promote diuresis in conditions like acute renal failure, prevent and treat oliguria.
Uses & Indications
Dosage
Adults
Typically 0.5-2 g/kg body weight, administered as a 15-20% solution by IV infusion over 30-60 minutes. Dosage varies based on indication and patient response.
Elderly
Lower initial doses and slower infusion rates may be considered due to potential for renal impairment.
Renal_impairment
Use with extreme caution. A test dose of 0.2 g/kg over 3-5 minutes may be given. If diuresis does not occur, Osmosol should not be administered.
How to Take
Administered by slow intravenous infusion through a peripheral or central venous catheter. Must be filtered prior to administration using a filter to ensure removal of any crystals.
Mechanism of Action
Osmosol increases the osmotic pressure of the glomerular filtrate, inhibiting tubular reabsorption of water and electrolytes, leading to increased excretion of water, sodium, and chloride. It also draws water from extravascular compartments, such as the brain and eye, into the vascular system, reducing edema and pressure.
Pharmacokinetics
Onset
Intracranial pressure reduction: 15-30 minutes. Diuresis: 1-3 hours.
Excretion
Primarily excreted unchanged by the kidneys. Renal clearance is directly proportional to glomerular filtration rate.
Half life
Approximately 2-4 hours, longer in renal impairment.
Absorption
Administered intravenously, so absorption is 100%.
Metabolism
Minimally metabolized; some small amount may be metabolized in the liver.
Side Effects
Contraindications

- •Severe pulmonary congestion or frank pulmonary edema

Drug Interactions
Lithium
Increased renal clearance of lithium, leading to decreased lithium levels.
Diuretics
Potentiates diuretic effect; may require dose adjustment.
Cyclosporine
May increase cyclosporine levels.
Nephrotoxic drugs
Increased risk of renal impairment.
Storage
Store at controlled room temperature (20-25°C). Do not freeze. Protect from light. Crystallization may occur at low temperatures; warm to dissolve.
Overdose
Symptoms include severe fluid and electrolyte disturbances (e.g., hypernatremia, hyponatremia, hyperkalemia, hypokalemia), acute renal failure, increased intracranial pressure (rebound effect), and heart failure due to fluid overload. Treatment involves discontinuing Osmosol, correcting electrolyte imbalances, and supportive care.
Pregnancy & Lactation
Pregnancy category C. Use only if potential benefit justifies the potential risk to the fetus. Caution advised during lactation; consult physician.
