Description
Overview of the medicine
Ostocare D is a combination medicine containing Calcium Carbonate and Vitamin D3, primarily used to treat and prevent calcium and vitamin D deficiencies, and to maintain bone health.
Uses & Indications
Dosage
Adults
Typically 1-2 tablets daily, or as directed by a physician.
Elderly
Same as adult dosage, but may require closer monitoring for potential interactions or hypercalcemia.
Renal_impairment
Use with caution and dose adjustment may be necessary in patients with mild to moderate renal impairment. Contraindicated in severe renal impairment.
How to Take
Take the tablet orally, preferably with a meal to enhance absorption. Do not crush or chew the tablet.
Mechanism of Action
Calcium Carbonate provides elemental calcium essential for bone formation and maintenance. Vitamin D3 (Cholecalciferol) facilitates the absorption of calcium from the gut and regulates calcium and phosphate levels in the body, promoting proper bone mineralization.
Pharmacokinetics
Onset
Gradual, effects typically seen with long-term consistent use.
Excretion
Calcium is excreted via urine and feces. Vitamin D metabolites are primarily excreted in bile and feces, with a small amount in urine.
Half life
Vitamin D3 has a half-life of approximately 19-48 hours.
Absorption
Calcium Carbonate is absorbed in the small intestine, with absorption enhanced by Vitamin D. Vitamin D3 is absorbed from the small intestine, primarily in the jejunum, and then transported via lymphatic system.
Metabolism
Vitamin D3 is hydroxylated in the liver to 25-hydroxyvitamin D (calcifediol) and then in the kidneys to the active form, 1,25-dihydroxyvitamin D (calcitriol). Calcium is largely excreted unchanged.
Side Effects
Contraindications
- Hypercalcemia (high calcium levels in the blood).
- Hypervitaminosis D (excessive vitamin D levels).
- Severe renal impairment or kidney stones.
- Hypersensitivity to any component of the product.
Drug Interactions
Digoxin
Hypercalcemia due to Vitamin D may increase the risk of digitalis toxicity.
Levothyroxine
Calcium may reduce levothyroxine absorption; administer at least 4 hours apart.
Bisphosphonates
Calcium can interfere with bisphosphonate absorption; separate administration by at least 30 minutes to 2 hours.
Corticosteroids
May decrease calcium absorption.
Thiazide diuretics
May reduce calcium excretion, leading to hypercalcemia.
Tetracyclines & Quinolone antibiotics
Calcium can impair absorption of these antibiotics; separate administration by at least 2-3 hours.
Storage
Store in a cool, dry place below 30°C, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include hypercalcemia (nausea, vomiting, constipation, thirst, polyuria, muscle weakness, confusion) and hypervitaminosis D. Treatment involves discontinuing the product, hydration, and medical management to lower calcium levels.
Pregnancy & Lactation
Can be used during pregnancy and lactation under medical supervision, as calcium and vitamin D requirements are increased. However, avoid excessive doses.
