Description
Overview of the medicine
Ostocare DX is a combination of Calcium Carbonate and Vitamin D3 (Cholecalciferol) used to prevent or treat calcium and vitamin D deficiencies, and to support bone health.
Uses & Indications
Dosage
Adults
1-2 tablets daily, or as directed by a physician. The tablets should preferably be taken after meals.
Elderly
Same as adult dosage, but caution should be exercised in patients with renal impairment.
Renal_impairment
Use with caution and monitor calcium levels. Dosage adjustment may be required.
How to Take
Oral administration. Tablets can be chewed or swallowed whole with water. Preferably taken after meals to enhance absorption and reduce gastrointestinal upset.
Mechanism of Action
Calcium Carbonate provides elemental calcium necessary for bone formation, nerve impulse transmission, muscle contraction, and maintaining normal cardiac function. Vitamin D3 facilitates the absorption of calcium from the gut and its incorporation into bone, helping to maintain proper calcium and phosphate levels.
Pharmacokinetics
Onset
Therapeutic effects on bone mineral density are gradual and observed over weeks to months with consistent use.
Excretion
Unabsorbed calcium is excreted in feces. Absorbed calcium is excreted primarily via urine, with some biliary excretion. Vitamin D metabolites are excreted primarily in bile and feces, with some renal excretion.
Half life
Calcium is continuously regulated. Vitamin D3's half-life can range from 15-30 days for calcifediol 25-hydroxyvitamin D, while the active form calcitriol 1,25-dihydroxyvitamin D has a half-life of 4-6 hours.
Absorption
Calcium carbonate is absorbed in the small intestine, primarily via active transport and paracellular diffusion. Vitamin D3 is absorbed from the small intestine, primarily with dietary fats, and requires bile salts for optimal absorption.
Metabolism
Calcium is not metabolized in the traditional sense; it is utilized by the body. Vitamin D3 is hydroxylated in the liver to 25-hydroxyvitamin D calcifediol and then in the kidneys to 1,25-dihydroxyvitamin D calcitriol, the active form.
Side Effects
Contraindications
- Hypersensitivity to calcium carbonate or Vitamin D3
- Hypercalcemia
- Severe hypercalciuria
- Nephrolithiasis
- Hypervitaminosis D
Drug Interactions
Bisphosphonates
Reduced absorption if taken concurrently. Administer calcium/Vit D after 30 minutes to 2 hours.
Corticosteroids
May decrease calcium absorption.
Thyroid hormones
Calcium can impair absorption of levothyroxine. Separate administration by at least 4 hours.
Thiazide diuretics
May reduce calcium excretion, leading to hypercalcemia.
Tetracyclines & Fluoroquinolones
Calcium can reduce the absorption of these antibiotics. Take at least 2-3 hours apart.
Storage
Store in a cool and dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include hypercalcemia nausea, vomiting, constipation, abdominal pain, muscle weakness, fatigue, mental disturbances, polydipsia, polyuria. Severe cases may lead to cardiac arrhythmias and coma. Treatment involves discontinuation of the supplement, hydration, and medical management of hypercalcemia.
Pregnancy & Lactation
Generally considered safe for use during pregnancy and lactation when calcium and vitamin D supplementation is indicated, provided recommended doses are not exceeded. Consult a physician.
