Overview of the medicine
Ostocurb is a dietary supplement containing Calcium Carbonate and Vitamin D3, used to prevent and treat calcium and vitamin D deficiencies, and to support bone health.
Uses & Indications
Dosage
Adults
Typically 1-2 tablets daily, or as directed by a physician. Best taken with meals.
Elderly
Same as adult dose, but caution is advised in elderly patients with pre-existing renal impairment.
Renal_impairment
Use with caution. Dose adjustment may be necessary based on serum calcium and phosphate levels. Avoid in severe renal impairment.
How to Take
To be taken orally. Tablets can be swallowed whole with water. It is generally recommended to take with food to enhance calcium absorption and minimize gastrointestinal upset.
Mechanism of Action
Calcium is an essential mineral for bone formation, maintenance, and various physiological functions. Vitamin D3 (Cholecalciferol) helps regulate calcium and phosphate homeostasis, promoting calcium absorption from the gut and its deposition in bones.
Pharmacokinetics
Onset
Long-term effect for bone health and deficiency correction. Not an immediate onset for acute symptoms.
Excretion
Calcium is primarily excreted via the kidneys (urine) and feces. Vitamin D3 and its metabolites are mainly excreted in bile and feces, with a small amount in urine.
Half life
Calcium has a relatively short plasma half-life (hours). Vitamin D3's active metabolites have a longer half-life, with 25-hydroxyvitamin D3 having a half-life of 15-30 days, and 1,25-dihydroxyvitamin D3 about 4-6 hours.
Absorption
Calcium absorption from the gut is variable (approx. 20-40%) and influenced by Vitamin D status. Vitamin D3 is well absorbed from the gastrointestinal tract, primarily in the small intestine.
Metabolism
Calcium is not metabolized. Vitamin D3 is hydroxylated in the liver to 25-hydroxyvitamin D3 (calcifediol), then further hydroxylated in the kidneys to 1,25-dihydroxyvitamin D3 (calcitriol), the active form.
Side Effects
Contraindications
- •Hypersensitivity to Calcium Carbonate, Vitamin D3, or any component of the formulation
- •Hypercalcemia (abnormally high levels of calcium in the blood)
- •Severe hypercalciuria (excessive calcium in urine)
- •Severe renal impairment
- •Nephrolithiasis (kidney stones) or history of recurrent kidney stones
- •Vitamin D toxicity
Drug Interactions
Bisphosphonates
Calcium can interfere with the absorption of bisphosphonates. Administer at least 30 minutes to 2 hours apart.
Corticosteroids
Can reduce calcium absorption and increase calcium excretion.
Thiazide diuretics
May reduce calcium excretion and increase the risk of hypercalcemia.
Cardiac glycosides (Digoxin)
Hypercalcemia increases the risk of digoxin toxicity. Monitor calcium levels carefully.
Thyroid hormones (Levothyroxine)
Calcium can reduce the absorption of thyroid hormones. Separate administration by at least 4 hours.
Tetracyclines and Fluoroquinolones
Calcium can chelate these antibiotics, reducing their absorption. Administer at least 2-3 hours before or 4-6 hours after calcium supplements.
Storage
Store in a cool, dry place, below 30°C, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose (hypercalcemia) may include nausea, vomiting, constipation, abdominal pain, thirst, polyuria (frequent urination), muscle weakness, fatigue, headache, and in severe cases, cardiac arrhythmias and renal impairment. Management involves discontinuing the supplement, ensuring adequate hydration, and seeking medical attention. In severe cases, IV fluids, diuretics, and corticosteroids may be necessary.
Pregnancy & Lactation
Ostocurb is generally considered safe and often recommended during pregnancy and lactation to ensure adequate calcium and Vitamin D intake, which are crucial for fetal and infant bone development. However, dosage should be determined by a healthcare professional.
