Description
Overview of the medicine
Otonix 20 mg tablet contains Olanzapine, an atypical antipsychotic used for the treatment of schizophrenia and bipolar I disorder.
Uses & Indications
Dosage
Adults
Schizophrenia: Initially 10 mg once daily, adjusted based on clinical response to 5-20 mg/day. Bipolar Mania: Initially 10-15 mg once daily, adjusted to 5-20 mg/day. Maintenance in Bipolar Disorder: 10-20 mg once daily.
Elderly
A lower starting dose (e.g., 5 mg/day) may be considered, with gradual titration under close monitoring due to increased sensitivity.
Renal_impairment
No specific dosage adjustment is usually required, but caution is advised in patients with severe renal impairment. Monitor clinical response.
How to Take
Otonix 20 mg tablets are for oral administration. They can be taken once daily with or without food. Swallow the tablet whole with water.
Mechanism of Action
Olanzapine is a selective monoaminergic antagonist with high affinity for serotonin 5-HT2A/2C, 5-HT6, dopamine D1-4, muscarinic M1-5, histamine H1, and adrenergic α1 receptors. It is believed to mediate its therapeutic effects through a combination of dopamine and serotonin antagonism.
Pharmacokinetics
Onset
Clinical effects typically observed within days to weeks of consistent treatment.
Excretion
Approximately 57% of the dose is excreted in urine and 30% in feces, mainly as inactive metabolites.
Half life
Mean terminal elimination half-life is approximately 21-54 hours (average 33 hours).
Absorption
Well absorbed after oral administration, peak plasma concentrations reached in approximately 5-8 hours. Bioavailability is not affected by food.
Metabolism
Extensively metabolized in the liver primarily by direct glucuronidation and cytochrome P450 mediated oxidation (CYP1A2 and CYP2D6).
Side Effects
Contraindications
- •Hypersensitivity to olanzapine or any component of the tablet
- •Patients with known narrow-angle glaucoma due to the anticholinergic effects of olanzapine
Drug Interactions
Anticholinergic Drugs
Potential for additive anticholinergic effects (e.g., constipation, dry mouth, urinary retention).
Antihypertensive Agents
May enhance the hypotensive effects of olanzapine, increasing the risk of orthostatic hypotension.
CYP1A2 Inhibitors (e.g., Fluvoxamine)
May increase olanzapine plasma concentrations, requiring a dose reduction.
CYP1A2 Inducers (e.g., Carbamazepine, Rifampin)
May decrease olanzapine plasma concentrations, potentially requiring a dose increase.
CNS Depressants (e.g., Alcohol, Benzodiazepines)
May have additive sedative effects.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include somnolence, slurred speech, tachycardia, extrapyramidal symptoms, respiratory depression, hypotension, and coma. Management involves supportive care, maintaining an open airway, oxygenation, and ventilation. Monitor cardiovascular function, and treat symptoms as appropriate.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Avoid during breastfeeding as olanzapine is excreted in breast milk.
