Description
Overview of the medicine
Otonix 40 mg Tablet is a proton pump inhibitor (PPI) used to reduce the amount of acid produced in the stomach. It is effective in treating gastroesophageal reflux disease (GERD), peptic ulcers, and other acid-related conditions.
Uses & Indications
Dosage
Adults
Usually 20-40 mg once daily, depending on the indication. For GERD, 20-40 mg once daily for 4-8 weeks. For Zollinger-Ellison syndrome, initial dose is 40 mg twice daily.
Elderly
No specific dose adjustment is generally required, but caution is advised in elderly patients with severe hepatic impairment.
Renal_impairment
No dose adjustment is necessary for patients with renal impairment.
How to Take
Take orally, preferably 30-60 minutes before a meal. Swallow the tablet whole; do not crush, chew, or split it.
Mechanism of Action
Esomeprazole works by irreversibly binding to and inhibiting the H+/K+-ATPase (proton pump) in the gastric parietal cells, thereby reducing gastric acid secretion.
Pharmacokinetics
Onset
Within 1 hour of oral administration.
Excretion
Excreted predominantly via the kidneys (80%) and in the feces (20%).
Half life
Approximately 1-1.5 hours.
Absorption
Rapidly absorbed after oral administration. Bioavailability is approximately 64% after a single dose and increases to 89% after repeated once-daily dosing.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 (CYP) enzyme system, specifically CYP2C19 and CYP3A4.
Side Effects
Contraindications
- •Hypersensitivity to esomeprazole, substituted benzimidazoles, or any component of the formulation.
- •Concomitant use with nelfinavir.
Drug Interactions
Digoxin
May increase digoxin absorption and levels.
Warfarin
May increase INR and risk of bleeding.
Clopidogrel
May reduce the antiplatelet effect of clopidogrel.
Methotrexate
May increase methotrexate levels and toxicity.
Nelfinavir, Atazanavir
Reduced plasma levels of antiretrovirals, avoid concomitant use.
Ketoconazole, Itraconazole
May decrease their absorption due to increased gastric pH.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose are generally mild and self-limiting. Management is symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category B. Consult your doctor before using during pregnancy. Use with caution during lactation as esomeprazole is excreted in breast milk.
