Description
Overview of the medicine
Oxat 10 mg Tablet contains Escitalopram, which is an antidepressant belonging to the Selective Serotonin Reuptake Inhibitor (SSRI) class. It is used to treat major depressive disorder and various anxiety disorders.
Uses & Indications
Dosage
Adults
Initial dose is 10 mg orally once daily. May be increased to a maximum of 20 mg daily after at least one week, based on clinical response.
Elderly
For patients >65 years, the initial dose is 5 mg orally once daily. Maximum recommended dose is 10 mg daily.
Renal_impairment
No dosage adjustment needed for mild to moderate renal impairment. Caution is advised in patients with severe renal impairment (creatinine clearance <20 mL/min).
How to Take
Take orally, with or without food, preferably at the same time each day.
Mechanism of Action
Escitalopram selectively inhibits the reuptake of serotonin (5-HT) in the brain, thereby increasing the concentration of serotonin in the synaptic cleft. This leads to enhanced serotonergic neurotransmission.
Pharmacokinetics
Onset
Antidepressant effects typically begin within 1-2 weeks, but full therapeutic effect may take 2-4 weeks or longer.
Excretion
Excreted primarily via the urine (about 8% as unchanged drug, and metabolites), with some fecal excretion.
Half life
Approximately 27-32 hours, allowing for once-daily dosing.
Absorption
Well absorbed from the gastrointestinal tract, not significantly affected by food. Peak plasma concentration occurs in about 3-4 hours.
Metabolism
Primarily hepatic, via CYP2C19, CYP2D6, and CYP3A4 enzymes to inactive or less active metabolites.
Side Effects
Contraindications
- •Hypersensitivity to escitalopram or any component of the formulation.
- •Concomitant use with Monoamine Oxidase Inhibitors (MAOIs), or within 14 days of discontinuing an MAOI.
- •Concomitant use with pimozide.
- •Congenital long QT syndrome or acquired QTc prolongation.
Drug Interactions
MAOIs
Risk of serotonin syndrome, which can be life-threatening.
Pimozide
Increased pimozide levels and potential for QTc prolongation.
Warfarin, NSAIDs, Aspirin
Increased risk of bleeding.
Other SSRIs/SNRIs, Tryptophan, St. John's Wort
Increased risk of serotonin syndrome.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include dizziness, tremor, nausea, vomiting, somnolence, tachycardia, and rarely seizures or serotonin syndrome. Management is primarily supportive, with maintenance of an open airway, oxygenation, and ventilation. Activated charcoal and gastric lavage may be considered.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Escitalopram is excreted in human milk; therefore, caution should be exercised when administered to a nursing woman.
