Overview of the medicine
Oxat 20 mg Tablet contains Escitalopram Oxalate, a selective serotonin reuptake inhibitor (SSRI) used to treat major depressive disorder (MDD), generalized anxiety disorder (GAD), social anxiety disorder, panic disorder, and obsessive-compulsive disorder (OCD). It works by increasing the level of serotonin in the brain, which helps to improve mood and reduce anxiety.
Uses & Indications
Dosage
Adults
Initial dose 10 mg once daily, may be increased to 20 mg once daily after at least one week, based on response. Maximum recommended dose is 20 mg/day.
Elderly
A lower initial dose (e.g., 5 mg once daily) and slower titration may be considered. Maximum recommended dose is 10 mg/day.
Renal_impairment
No dosage adjustment needed for mild to moderate renal impairment. Use with caution in severe renal impairment (CrCl < 20 mL/min).
How to Take
May be taken with or without food. Swallow the tablet whole with water.
Mechanism of Action
Escitalopram selectively inhibits the reuptake of serotonin (5-HT) in the presynaptic neuronal membrane, thereby increasing the concentration of 5-HT in the synaptic cleft. This leads to enhanced serotonergic neurotransmission.
Pharmacokinetics
Onset
Initial effects may be seen within 1-2 weeks; full antidepressant effect usually observed after 4-6 weeks.
Excretion
Excreted primarily via urine and feces.
Half life
Approximately 27-32 hours.
Absorption
Rapidly absorbed after oral administration. Peak plasma concentration achieved in about 3-4 hours.
Metabolism
Extensively metabolized in the liver, primarily by CYP3A4 and CYP2C19, to active and inactive metabolites.
Side Effects
Contraindications
- •Hypersensitivity to escitalopram or any component of the formulation.
- •Concomitant use with MAOIs (Monoamine Oxidase Inhibitors) or within 14 days of discontinuing MAOI treatment.
- •Concomitant use with pimozide.
Drug Interactions
MAOIs
Increased risk of serotonin syndrome.
NSAIDs
Increased risk of gastrointestinal bleeding.
Warfarin
May increase anticoagulant effect, requiring INR monitoring.
SSRIs/SNRIs
Increased risk of serotonin syndrome.
Tricyclic Antidepressants (TCAs)
May increase plasma concentrations of TCAs.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include dizziness, tremor, nausea, vomiting, somnolence, sinus tachycardia, and seizures. Management is supportive and symptomatic; maintain airway, monitor cardiac and vital signs. Activated charcoal may be considered.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Escitalopram is excreted into breast milk; caution advised during lactation.
