Description
Overview of the medicine
Ofloxacin is a broad-spectrum fluoroquinolone antibiotic used to treat various bacterial infections including those of the urinary tract, respiratory tract, skin, and sexually transmitted diseases.
Uses & Indications
Dosage
Adults
Typical dose is 200 mg to 400 mg every 12 hours, depending on the severity and type of infection. Treatment duration varies from 3 to 14 days.
Elderly
Dosage adjustment may be necessary based on renal function. Close monitoring is recommended.
Renal_impairment
For creatinine clearance 20-50 mL/min, administer the usual dose every 24 hours. For creatinine clearance less than 20 mL/min, administer usual initial dose, then 100 mg every 24 hours.
How to Take
Take orally, with or without food. Swallow the tablet whole with a glass of water. Do not crush, chew, or break the tablet.
Mechanism of Action
Ofloxacin inhibits bacterial DNA gyrase and topoisomerase IV, enzymes essential for bacterial DNA replication, transcription, repair, and recombination, leading to bactericidal action.
Pharmacokinetics
Onset
Onset of action varies depending on the type and severity of infection, generally within hours to days.
Excretion
Primarily excreted unchanged in the urine (70-80% of the dose) within 24-48 hours.
Half life
Approximately 5-8 hours in patients with normal renal function.
Absorption
Rapid and well absorbed from the gastrointestinal tract. Bioavailability is approximately 90-100%. Peak plasma concentrations are reached within 1-2 hours.
Metabolism
Minimally metabolized in the liver; about 5-10% of the dose undergoes metabolism.
Side Effects
Contraindications
- Hypersensitivity to ofloxacin, other quinolones, or any component of the formulation
- History of tendinitis or tendon rupture associated with fluoroquinolone use
- Epilepsy or other seizure disorders
- Children and adolescents (under 18 years) due to risk of arthropathy
- Patients with G6PD deficiency
Drug Interactions
Warfarin
May enhance the anticoagulant effects of warfarin, requiring close monitoring of INR and prothrombin time.
Theophylline
May increase plasma concentrations of theophylline, leading to increased risk of theophylline-related adverse effects. Monitor theophylline levels.
Corticosteroids
Concomitant use may increase the risk of tendon rupture.
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
Concurrent use may increase the risk of CNS stimulation and convulsive seizures.
Antacids (containing magnesium, aluminum), Sucralfate, Iron, Zinc
These agents can reduce the absorption of Ofloxacin. Administer Ofloxacin at least 2 hours before or 2 hours after these products.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, symptoms may include CNS effects (confusion, dizziness, seizures), gastrointestinal reactions (nausea, vomiting), and renal impairment. Management includes gastric lavage, activated charcoal, and symptomatic and supportive treatment. Maintain adequate hydration.
Pregnancy & Lactation
Pregnancy Category C. Not recommended during pregnancy due to potential effects on fetal cartilage development observed in animal studies. Ofloxacin is excreted in human breast milk; therefore, it is not recommended during lactation due to potential risks to the infant.
