Description
Overview of the medicine
Olanzapine is an atypical antipsychotic medication used to treat symptoms of schizophrenia and bipolar disorder. It helps to restore the balance of certain natural substances (neurotransmitters) in the brain.
Uses & Indications
Dosage
Adults
Schizophrenia: Initially 10 mg once daily. Dosage range 5-20 mg/day based on clinical response. Bipolar mania: Initially 10-15 mg once daily. Adjust dosage as clinically indicated.
Elderly
A lower starting dose of 5 mg/day may be considered for patients 65 years or older, especially if other risk factors are present.
Renal_impairment
No routine dosage adjustment is required for mild to moderate renal impairment, but caution is advised in severe cases.
How to Take
Administer orally, once daily, with or without food. Tablets can be swallowed whole with water.
Mechanism of Action
Olanzapine acts as an antagonist at multiple receptors including dopamine D1, D2, D3, D4, serotonin 5-HT2A, 5-HT2C, 5-HT3, 5-HT6, muscarinic M1-M5, and alpha-1 adrenergic receptors. This broad receptor binding profile contributes to its therapeutic effects in managing psychotic and mood symptoms.
Pharmacokinetics
Onset
Initial therapeutic effects may be observed within a few days, but full antipsychotic effects may take several weeks.
Excretion
Primarily urine (57%) and feces (30%).
Half life
Mean half-life is 21-54 hours (average 33 hours), allowing for once-daily dosing.
Absorption
Well absorbed after oral administration, peak plasma concentrations reached in 5-8 hours. Food does not affect the rate or extent of absorption.
Metabolism
Primarily hepatic, via direct glucuronidation and cytochrome P450-mediated oxidation (CYP1A2 and CYP2D6).
Side Effects
Contraindications
- Known hypersensitivity to Olanzapine or any component of the formulation.
- Patients with known narrow-angle glaucoma.
Drug Interactions
Fluvoxamine
Concurrent use may increase olanzapine levels due to CYP1A2 inhibition.
Carbamazepine
May decrease olanzapine levels due to CYP1A2 induction.
Alcohol/CNS Depressants
Additive sedative effects; avoid concurrent use or use with caution.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include tachycardia, agitation, dysarthria, sedation, coma, and extrapyramidal symptoms. Management is supportive, including airway management, activated charcoal, and monitoring of cardiovascular function and vital signs. There is no specific antidote.
Pregnancy & Lactation
Pregnancy: Use only if the potential benefit justifies the potential risk to the fetus. Lactation: Not recommended, as olanzapine is excreted in human milk and may cause adverse effects in a nursing infant.
