How to Take
Oral administration. For infants and young children, the tablet can be dissolved in a small amount of water (e.g., 5-10 ml) or chewed if age-appropriate. Dosage is typically 10-15 mg/kg body weight per dose, every 4-6 hours as needed, not exceeding 60 mg/kg/day or 4 doses in 24 hours.
Mechanism of Action
Paracetamol is thought to act primarily in the central nervous system, inhibiting prostaglandin synthesis by blocking cyclooxygenase (COX) enzymes, particularly COX-2. Its antipyretic effect is due to direct action on the hypothalamic heat-regulating center, increasing peripheral vasodilation and sweating.

Excretion
Mainly excreted in urine as glucuronide and sulfate conjugates. Less than 5% is excreted unchanged.
Half life
1 to 3 hours (adults), slightly shorter in children.
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract, with peak plasma concentrations occurring within 10 to 60 minutes after oral administration.
Metabolism
Primarily metabolized in the liver via glucuronidation and sulfation pathways. A small amount is metabolized by cytochrome P450 into a highly reactive intermediate (NAPQI).

Drug Interactions
Alcohol

Metoclopramide, Domperidone
May increase the absorption rate of paracetamol.

Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Overdose can cause severe, potentially fatal liver damage. Symptoms may include nausea, vomiting, abdominal pain, loss of appetite, and jaundice. Seek immediate medical attention. Treatment involves gastric lavage, activated charcoal, and N-acetylcysteine (NAC) administration.
