Overview of the medicine
Pacet 200 mg Tablet contains Paracetamol, which is a widely used medication for relieving mild to moderate pain and reducing fever. It works by blocking chemical messengers in the brain that tell us we have pain and by reducing the body's temperature.
Uses & Indications
Dosage
Adults
The usual dose for adults is 500 mg to 1000 mg every 4 to 6 hours as needed, not exceeding 4000 mg (4 grams) in 24 hours. For 200 mg tablets, dose adjustment may be required.
Elderly
No specific dose adjustment is usually required for elderly patients, but caution should be exercised in those with impaired renal or hepatic function.
Renal_impairment
In severe renal impairment (creatinine clearance < 10 mL/min), the dosing interval should be extended to 8 hours.
How to Take
Pacet 200 mg Tablet should be taken orally with water, with or without food. Do not exceed the recommended dose or frequency.
Mechanism of Action
Paracetamol is thought to act primarily in the central nervous system, inhibiting prostaglandin synthesis by selectively inhibiting cyclooxygenase (COX) activity, particularly COX-2. It reduces fever by direct action on the hypothalamic heat-regulating center, resulting in peripheral vasodilation and increased heat loss.
Pharmacokinetics
Onset
Analgesia: 30-60 minutes; Antipyresis: <60 minutes.
Excretion
Mainly excreted in the urine as glucuronide and sulfate conjugates. Less than 5% is excreted unchanged.
Half life
1-4 hours (typically 2-3 hours) in adults; slightly shorter in children, slightly longer in neonates.
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract, with peak plasma concentrations occurring within 30-60 minutes.
Metabolism
Primarily metabolized in the liver via glucuronide and sulfate conjugation. A small amount is metabolized by cytochrome P450 enzymes to a highly reactive intermediate (NAPQI), which is rapidly detoxified by conjugation with glutathione.
Side Effects
Contraindications

Drug Interactions
Alcohol
Chronic alcohol intake may increase the risk of liver toxicity when taking Paracetamol.
Cholestyramine
Reduces the absorption of Paracetamol if given within one hour.
Warfarin and other oral anticoagulants
May increase the anticoagulant effect, leading to an increased risk of bleeding, especially with prolonged and high doses of Paracetamol.
Phenytoin, Carbamazepine, Barbiturates (enzyme inducers)
May increase the metabolism of Paracetamol, potentially leading to increased risk of hepatotoxicity.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture.
Overdose
Symptoms of overdose include nausea, vomiting, abdominal pain, and liver damage (which may manifest several days later). Immediate medical attention is crucial. Treatment includes gastric lavage and administration of activated charcoal and N-acetylcysteine (NAC) as an antidote.
Pregnancy & Lactation
Paracetamol is generally considered safe for use during pregnancy and lactation at recommended therapeutic doses, but it is always advisable to consult a doctor before use.
