Overview of the medicine
Painac-TR provides strong relief from moderate to severe pain by combining anti-inflammatory, antipyretic, and opioid analgesic effects.
Uses & Indications
Dosage
Adults
One tablet 2-3 times daily or as directed by physician.
Elderly
Lower doses may be required, closely monitor for adverse effects.
Renal_impairment
Dosage adjustment required, contra-indicated in severe impairment.
How to Take
Take orally with or without food. Swallow whole, do not crush or chew.
Mechanism of Action
Aceclofenac inhibits prostaglandin synthesis. Paracetamol inhibits prostaglandin synthesis in the CNS. Tramadol binds to μ-opioid receptors, inhibiting norepinephrine and serotonin reuptake.

Onset
30-60 minutes.
Excretion
Primarily renal.
Half life
Aceclofenac approx 4 hours, Paracetamol 1-4 hours, Tramadol 5-7 hours.
Absorption
Rapidly absorbed after oral administration.
Metabolism
Primarily hepatic.
Side Effects
Contraindications
- •Hypersensitivity to any component
- •Severe hepatic/renal impairment
- •History of GI bleeding or perforation
- •Asthma, urticaria, or allergic-type reactions after taking NSAIDs or aspirin
- •Acute alcohol, hypnotics, centrally acting analgesics, opioids, or psychotropic drugs intoxication
- •Opioid addiction
- •Epilepsy not adequately controlled by treatment
Drug Interactions
Warfarin
Increased bleeding risk.
Alcohol, CNS depressants
Enhanced CNS depression.
Other NSAIDs or Salicylates
Increased risk of GI adverse effects.
Lithium, Methotrexate, Digoxin
Increased plasma levels of these drugs.
Diuretics and Antihypertensives
Reduced efficacy of these drugs.
SSRIs, MAOIs, Tricyclic Antidepressants
Increased risk of Serotonin Syndrome and seizures.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Gastric lavage, activated charcoal, symptomatic and supportive treatment. Naloxone for tramadol overdose. N-acetylcysteine for paracetamol overdose.
Pregnancy & Lactation
Not recommended in pregnancy (especially third trimester due to NSAID component) and lactation due to risks to fetus/infant and potential for opioid dependence. Consult doctor.
