Description
Overview of the medicine
Paincare 500 mg tablet contains Paracetamol, which is a widely used medication for relieving pain and reducing fever. It works by blocking the production of certain chemical messengers in the brain that cause pain and fever.
Uses & Indications
Dosage
Adults
500 mg to 1000 mg every 4-6 hours as needed, not exceeding 4000 mg (4g) in 24 hours.
Elderly
No specific dose adjustment is usually required, but caution should be exercised in those with impaired renal or hepatic function.
Renal_impairment
For creatinine clearance 10-50 mL/min, administer every 6 hours; for creatinine clearance <10 mL/min, administer every 8 hours.
How to Take
Paincare 500 mg tablet should be taken orally with or without food. Swallow the tablet whole with water. Do not crush or chew.
Mechanism of Action
Paracetamol (acetaminophen) is believed to act primarily in the central nervous system, increasing the pain threshold by inhibiting cyclooxygenase (COX) enzymes, particularly COX-2, which are involved in prostaglandin synthesis. It also acts on the hypothalamic heat-regulating center, producing antipyresis.
Pharmacokinetics
Onset
Within 30 to 60 minutes
Excretion
Mainly excreted in the urine as glucuronide and sulfate conjugates. Less than 5% is excreted unchanged.
Half life
1.5 to 3 hours
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract, with peak plasma concentrations occurring within 30 to 60 minutes after oral administration.
Metabolism
Primarily hepatic via conjugation with glucuronic acid and sulfate. A small portion is metabolized by cytochrome P450 into a toxic intermediate, NAPQI, which is then detoxified by glutathione.

Increased anticoagulant effect, leading to increased risk of bleeding.
Other paracetamol-containing products
Increased risk of overdose and liver damage.
Phenobarbital, Phenytoin, Carbamazepine
