Pamix 500 mg tablet contains Paracetamol, which is used to relieve pain and reduce fever. It is suitable for a wide range of common aches and pains.
Uses & Indications
Dosage
Adults
1-2 tablets (500-1000 mg) every 4-6 hours as needed, not exceeding 4000 mg (8 tablets) in 24 hours.
Elderly
Similar to adult dosage, but caution with hepatic or renal impairment.
Renal_impairment
Dosage interval may need to be extended to 6-8 hours, depending on creatinine clearance.
How to Take
Take orally with water, with or without food.
Mechanism of Action
Paracetamol (Acetaminophen) is thought to act primarily in the central nervous system, inhibiting prostaglandin synthesis via COX-2 inhibition, leading to its analgesic and antipyretic effects. It has weak anti-inflammatory properties.
Pharmacokinetics
Onset
30-60 minutes for analgesia; 30 minutes for antipyresis.
Excretion
Mainly excreted in the urine as glucuronide and sulfate conjugates, with less than 5% excreted as unchanged paracetamol.
Half life
1-4 hours, typically 2-3 hours in adults.
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract, with peak plasma concentrations occurring within 30-60 minutes.
Metabolism
Primarily metabolized in the liver by conjugation with glucuronide and sulfate. A small amount is metabolized via the cytochrome P450 enzyme system to a toxic intermediate, N-acetyl-p-benzoquinone imine (NAPQI), which is normally detoxified by glutathione. In overdose, glutathione stores can be depleted.

Contraindications

Drug Interactions
Alcohol
Increases risk of hepatotoxicity with chronic alcohol consumption.
Warfarin
May enhance the anticoagulant effect of warfarin, increasing bleeding risk.
Cholestyramine
Reduces paracetamol absorption if taken within one hour of paracetamol.
Metoclopramide/Domperidone
Increases paracetamol absorption rate.
Storage
Store in a cool, dry place below 30°C, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include nausea, vomiting, abdominal pain, and liver damage (jaundice, right upper quadrant pain). Treatment involves gastric lavage, activated charcoal, and administration of N-acetylcysteine (NAC) as an antidote.
Pregnancy & Lactation
Generally considered safe for use during pregnancy and lactation at recommended doses, but consult a doctor.
