Description
Overview of the medicine
Pantoprazole is a proton pump inhibitor used to treat conditions involving excessive stomach acid, such as GERD, erosive esophagitis, and ulcers.
Uses & Indications
Dosage
Adults
For GERD/erosive esophagitis: 20 mg once daily for 4-8 weeks. Maintenance: 20 mg daily. Zollinger-Ellison Syndrome: Initial 40 mg twice daily, adjusted as needed.
Elderly
No dose adjustment generally required, but caution is advised in patients with severe hepatic impairment.
Renal_impairment
No dose adjustment is required for patients with renal impairment.
How to Take
Take orally, usually in the morning, before food. Swallow the tablet whole; do not crush, chew, or split it.
Mechanism of Action
Panotem (Pantoprazole) selectively blocks the H+/K+-ATPase enzyme system (proton pump) in gastric parietal cells, inhibiting the final step in gastric acid production.
Pharmacokinetics
Onset
Acid suppression starts within 2.5 hours, with maximum effect observed after repeated dosing.
Excretion
Primarily renal (approximately 80%) and biliary/fecal (approximately 20%).
Half life
Approximately 1 hour (terminal half-life).
Absorption
Rapidly absorbed after oral administration. Bioavailability is approximately 77% and is not affected by food.
Metabolism
Extensively metabolized in the liver via CYP2C19 and CYP3A4 enzymes.
Side Effects
Contraindications
- Hypersensitivity to pantoprazole, substituted benzimidazoles, or any component of the formulation.
- Co-administration with atazanavir or nelfinavir due to significant reduction in their absorption.
Drug Interactions
Warfarin
Increased INR, monitor closely.
Clopidogrel
Reduced antiplatelet effect, use with caution.
Methotrexate
Increased methotrexate levels, especially with high dose methotrexate.
Atazanavir, Nelfinavir
Reduced absorption, avoid co-administration.
Iron salts, Ketoconazole, Itraconazole
Reduced absorption due to increased gastric pH.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.

In case of overdose, symptomatic and supportive treatment should be initiated. Pantoprazole is not readily removed from the circulation by hemodialysis.
