Panotem 40 mg Tablet contains Pantoprazole, a proton pump inhibitor used to reduce the amount of acid produced in the stomach. It treats conditions like heartburn, acid reflux, and stomach ulcers by blocking the proton pump in the parietal cells of the stomach lining.
Uses & Indications
Dosage
Adults
For Erosive Esophagitis: 40 mg once daily for up to 8 weeks. For maintenance: 40 mg once daily. For Zollinger-Ellison Syndrome: Initial dose of 40 mg twice daily, adjusted as needed.
Elderly
No dosage adjustment necessary based solely on age, but consider overall health and comorbidities.
Renal_impairment
No dosage adjustment necessary for renal impairment.
How to Take
Take the tablet whole, preferably in the morning before a meal. Do not crush, chew, or split the tablet.
Mechanism of Action
Pantoprazole irreversibly binds to the H+/K+-ATPase (proton pump) in the secretory canaliculi of the gastric parietal cells, inhibiting gastric acid secretion. This inhibition is dose-dependent and leads to both basal and stimulated acid output reduction.
Pharmacokinetics
Onset
Antisecretory effect within 1 hour, maximum effect within 2-4 hours.
Excretion
Mainly excreted renally (approximately 80%) and partially through feces (approximately 18%).
Half life
Approximately 1 hour (terminal half-life).
Absorption
Rapidly absorbed after oral administration. Bioavailability is approximately 77%.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 system (primarily CYP2C19 and CYP3A4).
Side Effects
Contraindications
- Known hypersensitivity to Pantoprazole or any component of the formulation
- Concurrent use with atazanavir or nelfinavir
Drug Interactions
Warfarin
Increased INR and prothrombin time, monitor carefully.
Methotrexate
Increased and prolonged methotrexate levels, particularly with high-dose methotrexate.
Atazanavir, Nelfinavir
Reduced absorption and efficacy of these antiretrovirals due to increased gastric pH. Avoid concomitant use.
Iron salts, Ketoconazole, Itraconazole
Reduced absorption of drugs whose bioavailability is pH-dependent.

Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.

Symptoms are generally mild and may include drowsiness, tachycardia, nausea, headache, and abdominal pain. Treatment is symptomatic and supportive. Pantoprazole is not removed by hemodialysis.

Pregnancy Category B. Use only if clearly needed. Excreted in human milk, caution is advised for nursing mothers.
