Overview of the medicine
Pantac 20 mg tablet is a proton pump inhibitor (PPI) used to reduce the amount of acid produced in the stomach. It treats conditions like heartburn, acid reflux, and ulcers.
Uses & Indications
Dosage
Adults
For GERD/Erosive Esophagitis: 20-40 mg once daily for 4-8 weeks. For Duodenal/Gastric Ulcers: 40 mg once daily for 2-4 weeks. For H. pylori eradication: 40 mg twice daily for 7-14 days (with antibiotics).
Elderly
No specific dosage adjustment is generally required, but caution is advised in patients with severe hepatic impairment.
Renal_impairment
No dosage adjustment is necessary for patients with impaired renal function.
How to Take
Take the tablet whole, do not crush, chew, or split. It is usually taken once daily in the morning, before a meal.
Mechanism of Action
Pantoprazole irreversibly binds to and inhibits the H+/K+ ATPase (proton pump) in the gastric parietal cells, thereby blocking the final step of acid secretion.

Excretion
Mainly excreted renally (about 80%) and the remainder in feces (about 20%) via biliary excretion.

Approximately 1 hour
Absorption
Rapidly absorbed after oral administration, peak plasma concentrations reached in 2-2.5 hours. Bioavailability is about 77%.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 system (primarily CYP2C19 and CYP3A4).
Side Effects
Contraindications
- Hypersensitivity to pantoprazole, substituted benzimidazoles, or any component of the formulation.
Drug Interactions
Warfarin
Possible increased INR and bleeding risk; monitor closely.
Methotrexate
Increased plasma levels of methotrexate, especially with high doses; consider temporary discontinuation of PPI.
Ketoconazole, Itraconazole, Atazanavir
Reduced absorption due to increased gastric pH.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
