Overview of the medicine
Pantac 40 mg tablet contains Pantoprazole, which is a proton pump inhibitor (PPI) used to reduce the amount of acid produced in the stomach. It is effective in treating acid-related conditions like heartburn, acid reflux, and stomach ulcers.
Uses & Indications
Dosage
Adults
Erosive Esophagitis: 40 mg once daily for up to 8 weeks. Maintenance: 40 mg once daily. Pathological Hypersecretory Conditions: Initial 40 mg twice daily, adjusted based on individual needs. Duodenal/Gastric Ulcers: 40 mg once daily, typically for 2-4 weeks. H. pylori eradication: 40 mg twice daily for 7-14 days in combination with appropriate antibiotics.
Elderly
No dosage adjustment is generally required.
Renal_impairment
No dosage adjustment is generally required for patients with renal impairment.
Hepatic_impairment
For patients with severe hepatic impairment, a maximum dose of 40 mg once daily should not be exceeded.
How to Take
Take Pantac 40 mg tablet once daily, preferably in the morning before a meal. Swallow the tablet whole with water. Do not chew or crush it.
Mechanism of Action
Pantoprazole irreversibly blocks the H+/K+-ATPase enzyme system (the proton pump) in the gastric parietal cells. This inhibition prevents the final step of gastric acid secretion, leading to a reduction in both basal and stimulated acid production.
Pharmacokinetics
Onset
Onset of acid secretion inhibition occurs within 2.5 hours, with maximum effect within 2.5 hours.
Excretion
Primarily excreted renally (approximately 80%) and the remainder in feces (approximately 20%) via biliary excretion.
Half life
Approximately 1 hour (terminal elimination half-life), but duration of acid suppression is much longer due to irreversible binding to proton pump.
Absorption
Rapidly absorbed after oral administration; absolute bioavailability is about 77%. Food may delay absorption but does not affect the overall bioavailability.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 (CYP) enzyme system, mainly CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Known hypersensitivity to pantoprazole, substituted benzimidazoles, or any component of the formulation.
- Concomitant use with rilpivirine-containing products.
Drug Interactions
Warfarin
Possible increased INR and prothrombin time. Monitor closely.
Methotrexate
May increase serum levels of methotrexate, particularly high doses. Consider temporary PPI withdrawal.
Atazanavir, Nelfinavir, Rilpivirine
Reduced absorption due to increased gastric pH, leading to decreased efficacy of antivirals. Avoid concomitant use.
Iron salts, Ketoconazole, Itraconazole, Erlotinib
Absorption may be decreased due to reduced gastric acidity.

Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
In case of overdose, seek immediate medical attention. Management should be symptomatic and supportive. Pantoprazole is not extensively removed from the circulation by hemodialysis.

Pregnancy Category B. Use during pregnancy only if clearly needed and the potential benefits outweigh the potential risks. Pantoprazole is excreted in human milk; caution should be exercised when administered to a nursing mother.
