Description
Overview of the medicine
Panto 20 mg Tablet contains pantoprazole, a proton pump inhibitor (PPI) that reduces the amount of acid produced in your stomach. It is used for the treatment of acid-related diseases of the stomach and intestine.
Uses & Indications
Dosage
Adults
Usually 20 mg or 40 mg once daily, depending on the condition. For GERD, 20-40 mg once daily for 4-8 weeks. For H. pylori eradication, 40 mg twice daily for 7-14 days.
Elderly
No specific dose adjustment is usually required for elderly patients.
Renal_impairment
No dose adjustment is generally required for patients with renal impairment.
How to Take
Take the tablet orally, preferably 30-60 minutes before a meal. Swallow the tablet whole with water; do not crush or chew it.
Mechanism of Action
Pantoprazole is a substituted benzimidazole that inhibits gastric acid secretion by irreversibly blocking the H+/K+-ATPase (proton pump) in the parietal cells of the stomach. This leads to a dose-dependent reduction of both basal and stimulated acid secretion.
Pharmacokinetics
Onset
Acid-suppressive effect begins within 2.5 hours, with full effect achieved after a few days of consistent dosing.
Excretion
Mainly excreted renally (approximately 80%) as metabolites, with the remainder excreted in feces (approximately 20%).
Half life
Approximately 1 hour (terminal half-life), but the duration of acid suppression is much longer due to irreversible binding.
Absorption
Rapidly absorbed after oral administration, with peak plasma concentrations occurring within 2-2.5 hours. Bioavailability is approximately 77%.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 (CYP) enzyme system, primarily CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to pantoprazole or any other proton pump inhibitor or to any of the excipients.
- Severe hepatic impairment (caution).
Drug Interactions
Warfarin
May increase INR and prothrombin time, increasing risk of bleeding. Monitor INR.
Methotrexate
May increase serum levels of methotrexate, especially at high doses. Consider temporary withdrawal of PPI.
Atazanavir/Nelfinavir
Coadministration not recommended due to significantly reduced bioavailability of antiretrovirals.
Drugs dependent on gastric pH for absorption (e.g., Ketoconazole, Iron salts)
Pantoprazole may decrease absorption of these drugs.

Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.

There is no known specific antidote. In case of overdose, symptomatic and supportive treatment should be given.
