Description
Overview of the medicine
Pantoprazole 40 mg Tablet is a proton pump inhibitor used to treat conditions where there is too much acid in the stomach. It works by reducing the amount of acid produced.
Uses & Indications
Dosage
Adults
Typically 40 mg once daily for 4-8 weeks, or as directed by physician.
Elderly
No dosage adjustment generally required, but caution in severe hepatic impairment.
Renal_impairment
No dosage adjustment needed.
How to Take
Take whole with water, preferably in the morning before a meal. Do not chew or crush the tablet.
Mechanism of Action
Pantoprazole is a substituted benzimidazole that inactivates the H+/K+-ATPase (proton pump) in the parietal cells of the stomach, thereby suppressing gastric acid secretion.

Onset
Within 2.5 hours
Excretion
Mainly renal (approx. 80%) and fecal (approx. 20%).
Half life
Approximately 1 hour (terminal half-life), but duration of acid suppression is much longer.
Absorption
Rapidly absorbed, bioavailability approximately 77%.
Metabolism
Extensively metabolized in the liver via cytochrome P450 (CYP2C19 and CYP3A4).
Side Effects
Contraindications
- Hypersensitivity to pantoprazole or any component of the formulation
- Hypersensitivity to other substituted benzimidazoles
Drug Interactions
Warfarin
Increased INR/prothrombin time; monitor closely.
Methotrexate
Possible increased methotrexate levels; consider temporary PPI withdrawal.
Atazanavir, Nelfinavir
Reduced absorption due to altered gastric pH; avoid co-administration.
Iron salts, ketoconazole, itraconazole
Reduced absorption of drugs requiring acidic gastric pH.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.

Symptoms of overdose are generally mild. Treatment is symptomatic and supportive. Pantoprazole is not removed by hemodialysis.
